Objective:To preliminarily explore the distribution characteristics of chronic eczema syndrome elements,and the correlation between the manifestations of chronic eczema skin lesions and Traditional Chinese Medicine(TCM)syndrome elements,so as to provide theoretical basis and syndrome differentiation reference for the clinical diagnosis and treatment of chronic eczema.Methods:By reviewing the literature on etiology,pathogenesis and syndrome differentiation of chronic eczema,the research progress on etiology,pathogenesis and syndrome differentiation of chronic eczema in TCM since ancient times was reviewed.In addition,the clinical investigation form of chronic eczema was designed in combination with syndrome factor differentiation scale of syndrome elements.145 patients with chronic eczema were investigated,and general conditions,course of disease,symptoms and skin lesions were recorded,and syndrome elements were extracted.All the contents of the investigation were entered into the excel table,and statistical analysis and data mining were conducted with SPSS software to explore the clinical syndromes and distribution characteristics of syndrome elements of chronic eczema and their correlation with the manifestations of chronic eczema lesions.Results:According to the frequency of syndrome factors distribution,the distribution of viscera was from high to low,which were liver,spleen,kidney,mind,stomach,surface and cytoplasm.In order of frequency,the distribution of pathological syndromes is Yin deficiency,dampness,phlegm,heat,qi deficiency,blood deficiency,Yang deficiency,qi stagnation,Yang hyperactivity,external wind,jin deficiency,cold,blood stasis,and blood heat.The cluster analysis results divided the 21 syndromes into six categories:the first category is Yin deficiency,the second category is heat,the third category is phlegm and dampness,the fourth category is spleen,qi deficiency,Yang deficiency,Yang deficiency and blood deficiency,the fifth category is liver and qi stagnation,the sixth category is xinshen,Yang hyperactivity,kidney,external wind,surface,blood heat,fluid deficiency,stomach,cytoplasm,blood stasis and cold.The association rules of data mining were used to propose that the association with high confidence under the condition that the support degree was no less than 10%had mild pruritus?Yang deficiency,moderate itching?heat,severe itching?phlegm and liver;Light erythema?heat and blood deficiency,obvious redness?heat and phlegm,crimson?phlegm and qi deficiency;Fine scales and scratches?liver and qi deficiency and blood deficiency,clear white thin scales?heat and phlegm,more scales and scratches?heat and sputum;Slight infiltration?Qi deficiency,relatively hypertrophy?hypertrophy and mossy not obvious,obvious infiltration,hypertrophy,mossy?phlegm and heat,mild mossy,phlegm and liver and heat.Conclusion:The pathogenesis of chronic eczema involves cold,heat,deficiency,solid,Yin,Yang,Yin deficiency and viscera loss.Common syndromes include Yin deficiency syndrome(liver and kidney Yin deficiency,Yin deficiency with fire,Yin deficiency with blood loss),damp(phlegm)heat syndrome,spleen deficiency(spleen qi deficiency,spleen Yang deficiency,spleen deficiency with blood loss,etc.),liver stagnation and qi stagnation,and can be accompanied by heart and kidney incompatibility syndrome,wind attack on the surface,stomach heat syndrome,cold coagulation cyst syndrome.The statistical analysis of the present study results show that the chronic eczema skin lesion on the relationship with potential pathogenesis:mild itching potential pathogenesis of Yang deficiency,moderate itching potential pathogenesis is hot,potential pathogenesis of severe itching to phlegm,and liver,light potential pathogenesis of erythema is hot,blood deficiency,her lesions clearly red potential pathogenesis is heat,phlegm,spotted potential pathogenesis is phlegmy,deficiency of dark red.Phlegm and papules are very dense and the potential pathogenesis of fusion is heat.The potential pathogenesis of fine scales and scratches is liver,qi deficiency and blood deficiency,and the potential pathogenesis of obvious scales and scratches is heat and sputum.No correlation between papule and syndrome element was found in this study.Chronic eczema is difficult to be cured and the pathogenesis is mixed.It is necessary to combine multiple factors to differentiate syndrome as a whole in clinical practice,so as to effectively guide clinical treatment and improve curative effect.
