Meta-analysis Of The Clinical Efficacy Of Liuwei Dihuang Pills In The Treatment Of Primary Osteoporosis Of Liver And Kidney Yin Deficiency

Objective:To explore the clinical efficacy of Liuwei dihuang Wan in treating primary osteoporosis(POP)of liver kidney yin deficiency.Methods:To extract the key words of retrieval,to design scientific and effective literatureretrieval type,and to make full use of the calculating machine to retrieve the Chinese knowledge network database(CNKI),the Wan Fang database(WF),the VIP database(VIP)and the biomedical literature database(CBM).The initial time of each database retrieval is to establish the data.The time and cut-off time of the library were all in January 2018.Artificial retrieval ofLiuwei dihuang Wan(composed of the same,undefined dosage forms)for the treatment of primary osteoporosis(without a random,blind method),according to this study set a unified standard and exclusion criteria for literature screening.This study used review Manager 5.3 version software for data analysis.Document quality evaluation is based on the quality evaluation tool provided by Cochrane collaboration to carry out document evaluation.The ratio ratio,that is OR,is used for the count data,and the standard mean number difference is used,namely,SMD and their 95%confidence interval(Cl).The purpose of this study is to evaluate the effect of the six flavour Rehmannia Pill on the total(non sex)lumbar bone density,the bone mineral density of the femoral neck,the value of the radius bone density at the middle and far 1/3 junction,the overall clinical efficacy evaluation,the evaluation of the curative effect of the bone pain,and the lumbar bone mineral density for the postmenopausal osteoporosis patients.The effect of the value and the evaluation of the clinical efficacy.Result:A total of 18 articles were included(2 of them were controlled trials,and the other 16were randomized controlled trials),including 1401 subjects,and the test group(Liuwei dihuang Wan group)included 758 subjects,and 643 subjects were included in the control group.Data analysis results:1.the role of Liuwei dihuang Wan in treating primary osteoporosis of liver and kidney yin deficiency(regardless of sex):1.1 The bone mineral density of lumbar spine was significantly higher than that of the control group,that is,(P=0.67,I~2=0.67%),[SMD=0.26,95%Cl=(0.12,0.40)]Z=3.71,P=0.0002,which had statistical significance.1.2 The bone density of the femoral neck was significantly higher than that of the control group,that is,(P=0.80,I~2=0%),[SMD=0.48,95%Cl=(0.26,0.70)],Z=4.28,P=0.0001,with statistical significance.1.3 The radius bone density at the middle and far 1/3 junctions of the forearm increased compared with the bone mineral density of the control group,(P=0.003,I~2=83%),[SMD=0.73,95%Cl=(0.08,1.38)],Z=2.21,P=0.03,and had statistical significance.1.4 The clinical efficacy of the treatment group was better than that of the control group,that is,(P=0.97,I~2=0%),[OR=4.58,95%Cl=(3.04,6.88)],Z=7.30,P<0.00001,with statistical significance.1.5 In the evaluation of the curative effect of bone pain,there was no significant difference between the control group and the test group,but the advantages of the test group,namely(P=0.0007,I~2=82%),[OR=3.50,95%Cl=(0.80,15.40)],Z=1.66,P=0.10,were not statistically significant.1.6 The two groups of adverse reactions were not statistically significant(P=0.94,I~2=0%),[OR=1.20,95%Cl(0.39,3.68)),Z=0.31,P=0.76,but the results were slightly less than the experimental group,but the bias could be caused by the amount of samples and the dosage of drugs.2.The clinical effect of Six Ingredient Rehmannia Pill in treating primary osteoporosis of liver kidney yin deficiency:2.1 The BMD of lumbar spine was significantly higher than that of the control group,that is,(P=0.73,I~2=0%),[SMD=0.26,95%Cl=(0.06,0.46)],Z=2.49,P=0.01,with statistical significance.2.2 The clinical curative effect is better than that of the control group,that is,(P=0.99,I~2=0%),[OR=4.11,95%Cl=(1.85,9.13)],Z=3.48,P=0.0005,has statistical significance.Conclusion:According to the results of this study,we know that the six flavour rehmannia pill has a certain advantage in the treatment of primary osteoporosis of liver and kidney yin deficiency,and it can effectively improve the bone density of the patients(the radius bone density at the lumbar vertebrae,the neck of the femur,the forearm,the far 1/3 junction)and the clinical efficiency.:there was no statistical significance in the treatment of bone pain and adverse reactions after taking.However,due to the limitations of the sample size and methodology,the study results may be biased,so it is expected to have more high quality and large sample randomized controlled trials to be verified,but the results of this study are still of certain reference value.
