Development And Preliminary Verification Of The Diagnostic Scale For TCM Syndromes Of Non-alcoholic Fatty Liver Disease

Objective: According to the basic theory of Traditional Chinese Medicine(TCM)and the law of dialectical thinking,the TCM diagnostic scale for non-alcoholic Fatty liver disease(NAFLD)was developed by means of literature retrieval,mathematical statistics and mathematical models,to provide an objective and standardized thinking and method for Clinical Syndrome differentiation.Methods:On the basis of the Chinese Medical Syndrome scale for non-alcoholic Fatty liver disease(NAFLD)initially developed in the early stage,we further read the literature and consult relevant experts,correlation analysis,stepwise regression and factor analysis were used to screen the items of Syndrome elements,logistic discriminant analysis was used to score the four items,and ROC curve was used to establish the threshold of syndrome diagnosis,The TCM Syndrome diagnosis scale of non-alcoholic fatty liver disease was formed,and the reliability and validity of the scale were scientifically assessed.Finally,a small sample clinical test was taken to test the sensitivity,specificity and diagnostic accuracy of the scale.Results:The TCM syndrome diagnostic scale of non-alcoholic Fatty liver disease includes five syndromes: Stagnation of liver-qi and Spleen Deficiency Syndrome,damp-heat syndrome,stagnation of damp-turbidity Syndrome,phlegm-blood Stasis Syndrome and deficiency of spleen and Kidney Syndrome.The diagnostic threshold of stagnation of liver-qi and Spleen Deficiency Syndrome is 7 points,the diagnostic threshold of dampness-heat Syndrome was 8 points,the diagnostic threshold of dampness-turbid Syndrome was 10 points,the diagnostic threshold of phlegm-stasis Syndrome was 10 points,and the diagnostic threshold of spleen-kidney Deficiency Syndrome was 6 points The rating scale has a split-half reliability of 0.657 and Cronbach’s of 0.757.Four common factors were extracted to represent 80.805% of the cumulative variance.The sensitivity of the scale was 76% as 90.9%,the specificity was 71% as 93%,and the accuracy was 76%-92%.Conclusion:The scale of TCM Syndrome diagnosis of non-alcoholic Fatty liver disease(NAFLD)has been developed.The scale has good reliability and validity. Preliminary verification shows that the accuracy of the scale can provide Standard Syndrome diagnosis criteria for clinical syndrome differentiation,it can be used for non-alcoholic Fatty Liver Disease Clinical or Scientific Research Standardization Syndrome diagnosis.
