Objective:To investigate the incidence of coronary heart disease combined with sleep disorders and the relationship between different degrees of sleep disorders and TCM syndromes and coronary heart disease-related risk factors through clinical investigations,and to analyze the distribution of TCM syndromes associated with coronary heart disease and sleep disorders.TCM clinical treatment and prevention provide a basis for reference.Method:1.Taking patients with coronary heart disease as the research object,conduct clinical investigation through the PSQI?AIS?ISI to explore the incidence of coronary heart disease patients with sleep disorders.2.Using statistical methods to analyze the differences in biochemical indicators(low density lipoprotein cholesterol,hs-CRP,interleukin-6)between different degrees of sleep disorders.3.Based on the diagnostic criteria of"Chinese Medicine Diagnosis"and"Traditional Chinese Medicine"published by China Traditional Chinese Medicine Press in 2017 as the reference standard,collect the information of the four diagnoses of patients with coronary heart disease and sleep disorders,and analyze coronary heart disease with sleep The distribution of TCM syndromes in patients with disorders.4.Use single factor and multi-factor Logistic regression analysis to explore the age,gender,smoking,family history of coronary heart disease,PCI history,coronary artery disease number,history of hypertension,type 2 diabetes history and different degrees of sleep disorders and TCM syndromes Correlation.Result:1.General information:A total of 350 subjects were included in this study,including 50 patients with simple coronary heart disease(no sleep disturbance),with an average age of(61.86±6.62)years old,31 males and 19 females.There were 300patients with coronary heart disease and sleep disorders,182 males(60.67%)with an average age of(62.30±6.24)years,118 females with an average age of(63.22±6.26)years;84 patients with severe sleep disorders,moderate There were 98 cases of sleep disorders and 118 cases of mild sleep disorders;there was no statistical significance in the age of onset(P>0.05).The average BMI of 300 patients with coronary heart disease and sleep disorders was(23.02±2.34)kg/m~2,and the average BMI of female patients was(24.68±1.40)kg/m~2;the results were statistically significant after independent sample t test Difference(P<0.05).2.Correlation between sleep disorders and risk factors:The family history of coronary heart disease has no significant effect on the degree of sleep disorders(X~2=0.016,P>0.05).There was a difference in smoking history among patients with coronary heart disease and different degrees of sleep disturbance(X~2=32.764,P<0.05).There is a difference in the history of hypertension between patients with coronary heart disease and different degrees of sleep disorders(X~2=4.063,P<0.05).There was no significant difference in the history of type 2 diabetes between patients with coronary heart disease and different degrees of sleep disturbance(X~2=0.009,P>0.05).There was a significant difference in the number of coronary artery lesions between patients with coronary heart disease and different degrees of sleep disorders(X~2=23.745,P<0.05).There was a significant difference in the history of PCI surgery between patients with coronary heart disease and different degrees of sleep disturbance(X~2=37.630,P<0.05).3.The impact on biochemical indicators:LDL-C content is different in patients with coronary heart disease and different degrees of sleep disorders.LDL-C levels in patients with sleep disorders are significantly higher than those without sleep disorders,and patients with severe sleep disorders have mild to moderate sleep.The level of LDL-C in people with disabilities was elevated,which was statistically significant(P<0.05);the level of LDL-C in people with moderate to mild sleep disorders was not significantly different,and was not statistically significant(P>0.05).Those with sleep disorders had higher levels of IL-6 than those without sleep disorders,those with severe sleep disorders had higher levels of IL-6 than those with mild to moderate sleep disorders,and those with moderate sleep disorders had higher levels of IL-6 than those with mild sleep disorders.High,with statistical significance(P<0.05).Patients with moderate to severe sleep disorders had higher hs-CRP levels than those with mild sleep disorders and those without sleep disorders.Those with severe sleep disorders had higher levels of hs-CRP than those with moderate sleep disorders(P<0.05);There was no significant difference in hs-CRP level between those with mild sleep disorder and those without sleep disorder(P>0.05).4.Distribution of TCM syndromes:69 patients with coronary heart disease combined with sleep disorders have qi stagnation and blood stasis syndrome,43 qi and blood deficiency syndromes,41 qi deficiency and blood stasis syndromes,41 qi and yin deficiency syndromes,40 heart and blood stasis syndromes 32 cases,30 cases of stagnation syndrome of stagnation of phlegm,12 cases of syndrome of internal obstruction of phlegm and turbidity,10 cases of syndrome of cold coagulation and heart pulse,10 cases of deficiency of heart and kidney yin,6 cases of deficiency of heart and kidney yang,and 5 cases of syndrome of liver depression and stagnation of qi,2 cases of heart fire hyperactivity syndrome;the main syndrome types of severe sleep disorder:20 cases of qi and blood deficiency syndrome,16 cases of qi deficiency and blood stasis syndrome,15 cases of qi and yin deficiency syndrome,11 cases of qi stagnation and blood stasis syndrome;moderate sleep The main syndrome types of the impaired persons are:21 cases of qi stagnation and blood stasis syndrome,18cases of qi deficiency and blood stasis syndrome,15 cases of qi and blood deficiency syndrome,13 cases of qi and yin deficiency syndrome,13 cases of gallbladder depression and phlegm disturbance syndrome;mild sleep disturbance The main syndrome types were:37 cases of qi stagnation and blood stasis syndrome,18 cases of heart and blood stasis syndrome,12 cases of gall stagnation and phlegm disturbance syndrome,and 12 cases of Qi and Yin deficiency syndrome.After examination,there are differences in the distribution of TCM syndromes between different sleep disorders,which is statistically significant(X~2=38.895,P<0.05).5.Logistic regression analysis of risk factors and TCM syndromes:history of smoking(OR=2.654,95%CI:1.334-7.546),history of hypertension(OR=2.432,95%CI:1.321-10.524)and PCI history(OR=2.765,95%CI:2.032-9.664)has a significant correlation with the distribution of TCM syndromes.Conclusion:1.Male patients with coronary heart disease are more prone to sleep disorders than female patients;BMI of female patients is overweight.2.The history of hypertension,PCI surgery,coronary vascular lesions have significant correlation with the occurrence of sleep disorders in patients with coronary heart disease;history of hypertension,PCI surgery,smoking history are independent risk factors for the distribution of TCM syndromes.3.The levels of LDL-C,IL-6 and hs-CRP in patients with coronary heart disease and sleep disorders are higher than those without coronary heart disease.The more severe the sleep disorder,the more significant the increase.4.The distribution of TCM syndromes of patients with coronary heart disease and different degrees of sleep disorders are different,including qi stagnation and blood stasis syndrome,qi and blood deficiency syndrome,qi deficiency and blood stasis syndrome,qi and yin deficiency syndrome,heart and blood stasis syndrome,Cholesterol and phlegm disturbing syndromes are more common.With the aggravation of the degree of sleep disturbance,the syndromes of traditional Chinese medicine gradually change from empirical syndromes to virtual ones.
