Purpose:This clinical study used the instructor's long-term clinical experience to intervene in the dyslipidemia of spleen deficiency and phlegm turbidity,and to observe the clinical efficacy and safety of western medicine and the effect on intestinal flora,APN and LEP.Material and method: This clinical trial is based on the subject "Prevention of the Spleen" and internal and external treatment of traditional Chinese medicine to intervene in dyslipidemia.Clinical trial has been approved by the ethics committee of Liaoning University of Traditional Chinese Medicine.In October 2018,a single-center model was recruited in the outpatient department and inpatient department of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine in compliance with TC> 5.20 mmol/L,and 60 subjects with dialectical spleen deficiency and phlegm turbidity.60 non-dyslipidemia subjects,60 subjects with TC> 5.20 mmol/L.Set them as Chinese medicine intervention group,non-dyslipidemia group and control group.The 180 people were randomly distributed in the three groups A,B,and C for intervention treatment according to the order of their entry time.All three groups have signed informed consent before enrollment.All were given health education and liver function(AST,ALT),renal function(Scr,BUN),blood routine,urine routine,stool routine,and ECG safety tests.Among them,the Chinese medicine intervention group gave the instructor a long-term clinical treatment of spleen deficiency,phlegm and turbid dyslipidemia.The course of treatment was 8 weeks.The medication was taken in boiling water.The control group and non-dyslipidemia did not intervene except for health education.Changes in blood lipids(TC,TG,HDL-C,LDL-C)of the Chinese medicine intervention group and the control group after eight weeks of treatment.Detection with fully automatic biochemical analyzer.The Chinese medicine intervention group plus the measured group had the same set of safety indicators.Thirty people from each of the three groups were selected to represent the level of the entire group,and the changes in serum leptin and adiponectin levels were measured using the Elisa test.In addition,the feces of 30 subjects in each of the three groups were selected for high-throughput sequencing comparison of intestinal flora.All data were statistically analyzed using SPSS 22.0.Statistical tests wereperformed using two-sided tests.P?0.05 was considered statistically significant.In the evaluation of TCM symptoms,the evaluation of disease efficacy refers to the ?Traditional Chinese Medicine Industry Standard of the People's Republic of China-TCM Syndrome Diagnosis and Efficacy Standard?.For the standard of symptomatic efficacy,refer to ?Clinical Guidelines for New Chinese Medicines? using nimodipine method.Intestinal flora was detected by high-throughput sequencing technology in fecal specimens to analyze the diversity and abundance of microbial flora in the intestine.Results:1.Observation of TCM effect on spleen deficiency and phlegm turbidity type dyslipidemia in traditional Chinese medicine:After taking Chinese medicine compound for 8 weeks,TCM symptom score showed a total effective rate of 94.8%,compared with 23.6% in the control group.Various symptoms of the Chinese medicine intervention group improved significantly.2.Observation of regulating spleen and expectorant to regulate blood lipids: After giving spleen deficiency,phlegm and turbid dyslipidemia(TC>5.2mmol/L as standard)volunteers for8 weeks,the total effective rate of TC value was 74%.3.Observation on the effects of spleen and expectorant regulation of blood lipids on serum LEP and ADP:after treatment with traditional Chinese medicine compound,LEP decreased and ADP content increased,and the results were statistically significant.(P<0.05)4.Observation on the effects of spleen and expectorant method on blood lipids and intestinal flora:After the treatment with Chinese medicine compound,the abundance and diversity of intestinal flora increased,and the results were statistically significant.(P<0.05)5.Comparison of adverse reactions:In the intervention group,adverse reactions occurred in 6cases(5 cases of abdominal distention,1 case of stomachache,which could be relieved by itself).Before and after treatment,there were no abnormal changes in blood,urine,stool routine,liver and kidney function and ECG in the intervention group.Conclusion:1.Traditional Chinese medicine spleen and expectorant method has obvious advantages in improving clinical symptoms of patients with spleen deficiency,phlegm and turbid dyslipidemia compared with the control group,and the traditional Chinese medicinecompound is safe and reliable,which can be further studied and promoted.2.he spleen and expectorant method has a significant effect on blood lipid related indicators such as TC,LEP and ADP.For diseases related to abnormalities in lipid metabolism,energy metabolism,and glucose metabolism related to these indicators,it has a role in traditional Chinese medicine to "treat the spleen" to interfere with blood lipid abnormalities.Provide some guidance on treatment ideas.3.The spleen and expectorant can regulate the spleen deficiency,phlegm and turbid dyslipidemia,and can also improve the intestinal flora disorder,which provides clinical evidence for improving the spleen deficiency state from the perspective of intestinal microecology.
