A Survey Of Traditional Chinese Medical Physique Of Patients With Diabetic Peripheral Neuropathy And Analysis Of Related Indicators

Purpose:By observing the distribution of TCM constitution of DPN patients and combining the general information and various indicators of type 2 diabetes patients,the relationship between DPN and TCM constitution was discussed,and the potential factors of complicated DPN were analyzed,so as to better control the prognosis of the disease and achieve the purpose of prevention and treatment.Material and method: 1.From July 2018 to July 2019,300 diabetic peripheral neuropathy patients in the endocrine ward or outpatient department of the Affiliated Hospital of Liaoning University of traditional Chinese medicine were selected as the research objects.According to the criteria of constitution determination in the classification and determination of constitution of traditional Chinese medicine,the patients were assisted in the constitution questionnaire survey.2.Under the guidance of the investigator,the patients filled in the TCSS neuropathy quantification form according to their own senses.3.General data of patients were recorded,and clinical indicators such as hba1 c,islet function and blood lipid were collected.4.The above data were input into Excel 2019,and SPSS 22.0 was used for statistics and analysis.Results: 1.There were 547 times in each constitution,of which Qi deficiency was the most frequent(42.00%),followed by blood stasis(40.67%),phlegm dampness(24.00%),qi depression(22.00%),yin deficiency(21.00%).2.Among 300 DPN patients,105(35%)had simple constitution and 195(65%)had complex constitution.3.In simple constitution,the blood stasis was the most frequent(22.86%),followed by phlegm dampness(18.10%),yin deficiency(17.14%),Yang deficiency(13.33%).4.The comparison of the above four indicators of simple constitution showed that the BMI level of Yin deficiency quality was the lowest(P < 0.05),and that of phlegm and dampness quality was the highest(P < 0.05).The total cholesterol level of phlegm and dampness was significantly higher than that of blood stasis and Yin deficiency(P < 0.05),and the total cholesterol level of Yang deficiency was significantly higher than that of Yin deficiency(P < 0.05).Patients with Yang deficiency had the highest TCSS score(P < 0.05).5.In the compound constitution,the number of cases with two kinds of compound constitution is the most(82.56%),the frequency from high to low is respectively blood stasis quality + Qi deficiency quality(27.95%),blood stasis quality + qi depression quality(20.50%),Qi deficiency quality + Yin deficiency quality(14.91%),Qi deficiency quality + phlegm dampness quality(13.66%).6.Among the patients with blood stasis quality + Qi deficiency quality,Qi deficiency quality + phlegm dampness quality,males were more than females(P < 0.05).Among the patients with qi deficiency + Yin deficiency,females were more than males(P < 0.05).The majority of patients aged 41-65 years had blood stasis + qi depression quality(P < 0.05).The majority of patients over 65 years old had blood stasis + Qi deficiency(P < 0.05).In the course of 0-5 years,the majority of patients were qi deficiency + Yin deficiency(P < 0.05).The course of disease in 5-10 years with blood stasis + Qi depression quality of the majority of patients(P < 0.05);The course of the disease was more than 10 years with blood stasis + Qi deficiency of the majority of patients(P < 0.05).In the moderate stage,blood stasis + qi depression quality was the most common(P < 0.05).The majority of the severe cases were blood stasis + Qi deficiency(P < 0.05).7.According to the comparison of the above four composite constitution indexes,the BMI level of patients with Qi deficiency quality + Yin deficiency quality was the lowest(P < 0.05),and the BMI level of Qi deficiency quality + phlegm dampness quality was the highest(P < 0.05).The highest level of blood stasis quality + qi depression quality Hb A1 c was found(P < 0.05).The fasting c-peptide level of blood stasis quality + qi depression quality was the lowest(P < 0.05),and the fasting c-peptide level of Qi deficiency quality + phlegm dampness quality was the highest(P < 0.05).The total cholesterol level of Qi deficiency quality + phlegm dampness quality was the highest(P < 0.05).The TCSS score of blood stasis quality + Qi deficiency quality was the highest(P < 0.05).Conclusion: 1.In diabetic patients,Qi deficiency quality,blood stasis quality,phlegm dampness quality,qi depression quality and Yin deficiency quality may increase the risk of DPN.2.In patients with simple constitution,blood stasis quality,phlegm dampness quality,Yin deficiency quality and yang deficiency quality may increase the risk of DPN.3.In patients with complex constitution,when the constitution type is blood stasis + Qi deficiency,blood stasis + qi depression,Qi deficiency + Yin deficiency and Qi deficiency + phlegm dampness,the risk of DPN may be increased.4.In DPN patients,when the pure constitution is Yang deficiency,or the complex constitution is blood stasis + Qi deficiency,it may increase the severity of neuropathy.5.When the patient is male with a constitution of blood stasis + qi stagnation,qi deficiency + phlegm and dampness,or female with a constitution of qi deficiency + Yin deficiency,the risk of DPN may be increased.The risk of DPN may be increased when the patient is between 41 and 65 years old and has a constitution of blood stasis + qi stagnation,or when the patient is over 65 years old and has a constitution of blood stasis + qi deficiency.In addition,through the analysis of BMI,hba1 c,fasting c-peptide,total cholesterol and other relevant indicators,doctors can carry out more targeted prevention and treatment of DPN according to different patients.
