Correlation Study Between TCM Syndromes Of Coronary Heart Disease With Chronic Renal Insufficiency And Coronary Artery Disease

BackgroundCoronary heart disease(CHD)is a serious threat to human health.In China,with the development of society and the change of people's diet structure,the incidence and fatality rate of CHD continue to rise.The traditional risk factors for CHD are smoking,drinking,hypertension,diabetes,abnormal lipid metabolism,etc.,and in recent years,more and more"non-traditional" risk factors have been confirmed,such as early renal failure(RF).Traditional Chinese medicine(TCM)has unique advantages in the treatment of CHD and RF,but there is not enough research on the combination of CHD and RF.In view of the above,it is necessary to explore the characteristics of the TCM syndromes of CHD and the degree of coronary lesions in different renal function cases.ObjectiveIn order to explore the effects of RF on the distribution of TCM syndromes of disease with CHD,the occurrence of "endogenous collateral wind",the degree of coronary artery lesions and the function of the heart.Methods1.The subjects were admitted to the Department of Cardiology at Dongzhimen Hospital of Beijing University of Traditional Chinese Medicine from January 2019 to August 2019,and were diagnosed with CHD by coronary imaging examination.2.According to the improved Modification of Diet in Renal Disease(MDRD)formula,eGFR was calculated,and the patient was divided into two groups of normal renal function and abnormal renal function according to eGFR level.3.Design clinical questionnaire,record the general clinical situation of the subject,Four diagnostic methods of TCM,coronary imaging results,ultrasound cardiac testing and other auxiliary examination.4.Organize the collected clinical data into the database,and import SPSS20.0 statistical software for statistical analysis.Results1.This study was selected for 241 cases of CHD patients,of which 113 cases of normal renal function,abnormal renal function 128 cases.After x2 test or t test analysis,it was found that the two groups of patients had differences in age,the average age of the renal dysfunction group was about 3 years older than the average age of the normal renal function group,the difference was statistically significant(P=0.01),and there was no statistical difference in sex,hypertension history,diabetes history,blood lipid abnormality,smoking history,drinking history,etc.2.Objective indicators:two groups of patients in Cr,BNP,LVEF,Gensini integral,three lesions ratio,two groups have statistical differences(P<0.05),but two groups of patients in FPG,UA,TC,TG,LDL-C,HDL-C,single lesionratio ratio,double lesion ratio no statistical difference(P>0.05)3.The proportion of heart blood stasis syndrome in the normal group of renal function was significantly larger than that of the renal function abnormal group,and the proportion of phlegm blocking heart vessel syndrome and deficiency of Yang Qi Syndrome of the renal dysfunction group was significantly greater than that of the normal renal function group,the difference was statistically significant(P<0.05);And there is no significant difference between two groups of other TCM syndromes(P>0.05).4.The occurrence of "endogenous collateral wind":the proportion of "endogenous collateral wind" occurred in the abnormal renal function group is significantly higher than the normal renal function group,the difference was statistically significant(P<0.01).5.During hospitalization and prognosis:the blood transplant reconstruction rate of the renal dysfunction group was significantly higher than that of the normal renal function group,the difference was statistically significant(P<0.01),and the incidence of acute heart failure during hospitalization and the rate of 3 0-day re-hospitalization were also significantly higher than that of the normal renal function group,and the difference was statistically significant(P<0.05)Conclusion1.Compared with the normal renal function group,Phlegm blocking heart vessel syndrome and deficiency of Yang Qi Syndrome appeared more in the RF group.2.The relatively heavy Chinese medicine certificate type of coronary lesions in patients with CHD is Phlegm blocking heart vessel syndrome and deficiency of Yang Qi Syndrome,and the patients of heart blood stasis syndrome has the lightest coronary lesions.3.Compared with the normal renal function group,the coronary artery lesions and the degree of heart function damage are more serious in CHD with renal insufficiency group.4.Compared with the normal renal function group,the incidence of CHD "endogenous collateral wind",and adverse events during hospitalization and adverse prognosis are more severe in CHD with renal insufficiency group.
