Development And Verification Of A Diagnostic Scale For Kidney Essence Deficiency Syndrome In Diabetic Nephropathy

Objective:Professor Liu pointed out that the core pathogenesis of diabetic nephropathy is Jingsunluobi.Clinically.Tianjingtongluo to treat diabetic nephropathy is effective.The correct judgment of the syndrome of kidney essence deficiency is the key to the curative effect of this method.However,the quantitative diagnosis of the deficiency of kidney essence in diabetic nephropathy is still lacking,which is not conducive to the popularization and application of the method of filling the collaterals.This project is based on literature analysis,expert consultation,and clinical research screening,and through statistical calculations,the quantitative diagnosis scale for the deficiency of kidney essence in diabetic nephropathy is completed.Lay the foundation for the clinical application and research of Tianjigtongluo in diabetic nephropathy with deficiency of kidney essence.Methods:In this study,based on the standard procedures and methods of scale development,through literature search,expert consultation and clinical cross-sectional research,the diagnostic criteria for kidney essence deficiency syndrome in diabetic nephropathy will be formulated.The specific methods are as follows:1,Clarify the research content and set up a working group;2.Document retrieval,establish a pool of items for kidney essence deficiency syndrome;3.Conduct two rounds of expert argumentation based on the Delphi method,select items,and conduct symptomatic standardization training for group members 4.Include 126 patients with diabetic nephropathy to collect their four-diagnosis information,use frequency analysis method,correlation coefficient method,Cronbach alpha coefficient method to filter items,use factor analysis method to weight items,use ROC curve method to determine the threshold,use The quartile method is used to determine the degree of classification,obtain the "Diabetic Nephropathy Kidney Essence Deficiency Syndrome Diagnostic Scale",and evaluate the reliability and validity of the scale;5.Re-enroll 52 patients with diabetic nephropathy,and use the clinician's syndrome differentiation result as the diagnosis."Gold Standard",compare the syndrome differentiation results of the "Diabetic Nephropathy Kidney Essence Deficiency Syndrome Diagnostic Scale",and verify the diagnostic accuracy,sensitivity,and specificity of the scale.Results:In this study,the item pool was established through literature search,and the items were selected in a combination of subjective and objective methods.The related symptom items of the Diabetic Nephropathy Kidney Essence Deficiency Syndrome Diagnostic Scale were as follows:fatigue,waist and knee weakness,heel pain,forgetfulness,dizziness,tinnitus,Deafness,loose teeth,loss of libido.Through the determination of weights and thresholds,each symptom item is weighted,and the diagnostic threshold is calculated to be?38.5.The degree classification is determined by the quartile method.38.5?point<62 is considered mild,62?point<124.5 is medium Degree,score?124.5 is severe.The diagnostic scale for kidney essence deficiency syndrome of diabetic nephropathy is compiled,and the reliability and validity of the scale are good.Through clinical verification with a small sample,the sensitivity of the scale is 95.2%,the specificity is 80%,and the accuracy is 92.3%.Conclusion:In this study,through the comprehensive application of literature retrieval,expert argumentation,clinical research and other methods,the symptoms were objective,and the diagnostic scale for diabetic nephropathy kidney essence deficiency syndrome was developed.The sensitivity is 95.2%,the specificity is 80%,and the accuracy is 92.3%.Lay the foundation for the clinical application and research of Tianjigtongluo in diabetic nephropathy with deficiency of kidney essence.
