Background:In clinic, the operation should be selected for the treatment of hemorrhoids which are ineffective after conservative treatment. There are many surgical methods for mixed hemorrhoids with bleeding and prolapse. The combined application of the operation conforms to the minimally invasive trend of improving symptoms and protecting anal function. The postoperative complications and recurrence rate can not be ignored, and there is no unified operation at present.Automatic elastic thread hemorrhoids ligation and electrochemistry of copper ion are widely used in clinical because of their safety, effectiveness and minor trauma. In clinical application,it was found that the combination of the two methods could improve the clinical effect, keep their advantages on the basis of fixation and hemostasis, make up for the deficiencies, reduce the occurrence of postoperative complications, and expand the scope of surgical indications. Objective:The purpose of this study is to objectively evaluate the clinical efficacy of automatic elastic wire ligation combined with electrochemistry of copper ion and procedure for prolapse and hemorrhoids in the treatment of mixed hemorrhoids(accompanied by bleeding and prolapse symptoms).Comparing the advantages and disadvantages of the two surgical methods.Selecting a reasonable surgical method to reduce postoperative complications and improve the clinical efficacy. Methods:The 60 cases in this study were all from the inpatients diagnosed as mixed hemorrhoids(accompanied with bleeding and prolapse symptoms)in Guang’anmen Hospital,Chinese Academy of traditional Chinese medicine from May 2019 to November 2019.The patients were randomly divided into experimental group(30 cases)and control group(30 cases). The two groups were anesthetized with spinal anesthesia, and operated by the same doctor. The experimental group was treated with automatic elastic wire ligation combined with electrochemistry of copper ion,and the control group was treated with procedure for prolapse and hemorrhoids. The therapeutic effect, postoperative symptom score, postoperative complication score,one-month follow-up and adverse events of the two groups were compared comprehensively. The data were analyzed by spss25.0 statistical software,so as to evaluate the two groups’ surgical methods comprehensively and objectively. Results:There was no significant difference between the experimental group and the control group in the baseline data(gender, age, course of disease, the number of internal hemorrhoids and external hemorrhoids incision)(P>0.05).There was no significant difference in clinical efficacy(P>0.05).The Intraoperative hemorrhage of the experimental group was lower than that of the control group(P<0.05).There was no significant difference between the two groups in fecal blood score before operation and on the 1st,3rd,7th,14th and 21st days after operation(P>0.05).The stool blood days in the experimental group were lower than those in the control group, the difference was statistically significant(P<0.05).There was no significant difference in pain score between the experimental group and the control group(P>0.05).And there was no significant difference in anal edema score between the two groups(P>0.05).There was statistical difference between the two groups in the scores of anal distension and urgency(P<0.05).There was no significant difference in the postoperative urinary retention between the two groups(P<0.05).After one month follow-up,there was no significant difference in the scores of residual hemorrhoids between the two groups(P>0.05).There were no postoperative anorectal dysfunction and anal stenosis in the experimental group and the control group. Conclusion:In the treatment of mixed hemorrhoids with bleeding and prolapse symptoms, automatic elastic wire ligation combined with electrochemistry of copper ion and procedure for prolapse and hemorrhoids have the same clinical effect.Compared with the single minimally invasive operation, the combined operation can not only enhance the strengthening and hemostasis effect, but also broaden the scope of operation indications and reduce the possibility of postoperative complications. The operation is safe, effective, easy to operate, and the degree of anal function damage is small, which can be selected by clinicians, especially young doctors.
