Objective Tic disorder is one of the neuropsychiatric disorders in children.Traditional Chinese medicine does not explicitly propose the name of tic disorder.According to its symptoms and clinical characteristics,most of them are classified into the categories of"frightening wind" and "chilling".Based on many years of clinical experience,the instructor believes that the core pathogenesis of tic disorders is "liver wind internal movement,phlegm and blood stasis obstruction".This subject objectively evaluates the improvement of vocal twitch and aripiprazole in vocal twitch,exercise twitch,functional impairment,and total tic points through the YGTSS scale.The twitch symptoms themselves and the susceptibility of tic disorders to repeated recurrence and poor efficacy are important reasons for increasing the psychological burden of TD children.The anxiety disorder caused by this is one of the main problems of children with tic disorders.To compare the efficacy of Xifeng Zhidong Decoction and Aripiprazole in improving anxiety in children with tic disorders.This study introduced the serum brain neurotrophic factor as a laboratory indicator.By comparing the changes of brain-derived neurotrophic factor before and after treatment,we can initially understand whether serum BDNF participates in the pathogenesis of tic disorder,and the severity of tic disorder and serum BDNF concentration during the pathogenesis.Whether it is relevant.Compare the improvement of TCM syndromes between the two drugs before and after treatment,evaluate the safety of the two drugs,and provide a clinical basis for drug promotion.Method This study included 60 cases of TD children who met the standards from December 2018 to January 2020 in the Pediatrics and Brain Department of Dongzhimen Hospital of Beijing University of Chinese Medicine for 8 weeks of clinical treatment.A total of 53 cases were completed.The experiment In the process,4 cases fell off,and 3 cases were eliminated.The included subjects were divided into two groups,including 29 cases in the Chinese medicine group and 24 cases in the Western medicine group.Respectively using Xifeng Zhitong Formula Granules and Aripiprazole tablets as treatment methods,using statistical analysis to compare the Yale Comprehensive Tic Severity Scale(YGTSS),Child Anxiety Mood Disorder Screening List(SCARED),and tic disorder Observe the clinical efficacy of Xifeng Zhidong Formula Granules and Aripiprazole by TCM syndrome score scale and brain-derived neurotrophic factor changes.At week 0 and week 8,the hematuria and stool routine,liver and kidney function,and electrocardiogram of the enrolled TD children were tested to evaluate the safety of the two treatment options,and the occurrence of adverse events during the treatment was observed.Follow up at the 6th month to observe the stability of the curative effect.Result(1)After 8 weeks of treatment,the total effective rate of YGTSS in the Chinese medicine group was 75.9%and the inefficiency was 24.1%.The total effective rate of YGTSS in the western medicine group was 79.2%and the inefficiency was 20.8%.X2=0.082,P=0.075>0.05,between the two groups There is no statistical basis for the difference,which indicates that the efficacy of treating tic disorder Xifengzhixie granules and aripiprazole is similar in the short term.After 8 weeks of medication,the exercise twitch reduction rate,functional impairment reduction rate and total points reduction rate of the Chinese medicine group and the Western medicine group were P value>0.05,and the difference was not statistically significant.The tic twitch reduction rate P<0.05,the difference was statistically significant.(2)After 8 weeks of treatment,the SCARED reduction rate of the Chinese medicine group was(64.89±2.31)%,and the SCARED reduction rate of the Western medicine group was(45.83 ±2.99)%,P=0.010<0.05.The difference in SCARED reduction rate between the two groups was statistically significant significance.(3)P>0.05 before and after treatment of the main symptoms and symptomatic symptoms of traditional Chinese medicine in the Chinese medicine group and the Western medicine group,the difference was not statistically significant.The main symptoms of traditional Chinese medicine in the traditional Chinese medicine group were tic twitching and sleep disturbance,irritability,and diet.The difference after treatment was P<0.05,and the difference was statistically significant.The main symptoms of traditional Chinese medicine in the western medicine group were exercise tic and concurrent sleep restlessness,after treatment P<0.05,The difference was statistically significant.After 8 weeks of medication,the TCM syndrome reduction rate of the Chinese medicine group was(58.82±15.06)%,and the TCM syndrome reduction rate of the Western medicine group was(17.86±9.18)%,p=0.046<0.05,the TCM syndrome reduction rate between the two groups The difference was statistically significant.(4)Comparison of serum BDNF in the Chinese medicine group and Western medicine group after treatment,P>0.05,suggesting that there is no statistically significant difference in serum BDNF between the two groups after treatment.Comparison between groups P>0.05,suggesting that there was no statistically significant difference in serum BDNF between the two groups.The serum BDNF before treatment in the Chinese medicine group was 2.23±0.91,the serum BDNF after treatment was 3.56±0.59,the serum BDNF in the western medicine group was 1.83±0.63,the serum BDNF after treatment was 2.20±0.78,the serum BDNF in the traditional Chinese medicine group and the Western medicine group after 8 weeks of treatment Rise.There were no obvious abnormalities in the routine hematuria,liver and kidney function,and electrocardiogram of the TD children in the TD group at week 0 and 8 respectively.There were no adverse reactions during the treatment.Conclusion(1)In the short term,Xifengzhixiu Formula Granules has the same therapeutic effect as aripiprazole.Xifengzhixiu Formula Granules have an advantage in improving tic tic in children with TD;(2)It can relieve anxiety in TD children Emotional disorders have significant effect of Xifengzhixiu formula particles;(3)Xifengzhixiu formula particles have obvious effects in improving TCM syndromes;(4)Xifengzhixi formula particles have not seen before and after treatment for children with tic disorders Serum BDNF changes have a significant correlation.
