| Background:Cancer pain is one of the biggest burdens of cancer patients.The phenomenon of insufficient control of cancer pain is still relatively common in China.Cancer pain brings extremely serious psychosomatic influence to patients,and more than 50%of patients have suffered from cancer pain.Cancer pain not only brings great pain to the patient but also brings a burden to the family.If the cancer pain is not controlled,on the one hand,it will accelerate the development of the tumor,on the other hand,it will affect the patient’s sleep,appetite,immunity,aggravate the patient’s anxiety,depression and other bad emotions.If chronic pain is not solved,it will develop into intractable cancer pain,which is one of the important causes of suicide.The world health organization proposes a three-step analgesic plan for cancer pain.Although this solution can solve most of the clinical pain,its clinical application is limited due to its addiction and adverse reactions.In recent years,traditional Chinese medicine has played a certain role in clinic.Despite of pain Hua Chan element has been widely used in clinical treatment,but Hua Chan Su capsules in the treatment of pain of potential mechanism research,beta-endorphin in recent years and 5 hydroxytryptamine in pain,much attention has been paid to this topic will preliminary Hua Chan vegetarian capsules on treating effect,improve the quality of life situation,the improvement of opioid constipation and the determination of beta-endorphin,serotonin,and the relationship between HuaChan element treatment of pain and,to provide certain application reference value in treatment of pain.Objective:To observe and evaluate the clinical efficacy and safety of Hua Chan Su capsules in the treatment of cancer painResearch Methods:A prospective,parallel randomized control method was used to collect 60 patients with cancer pain who met the inclusion criteria.30 cases were randomly divided into the treatment group and the control group.Treatment group:in the control group,on the basis of taking drugs to relieve pain,oral administration of huapadin capsules(drugs produced and marketed by shaanxi dongtai pharmaceutical co.,LTD.),3 pills,3 times/day.At the end of the study,the effect of treatment,improvement of quality of life and opioid constipation were evaluated,and the levels of endorphins and serotonin were measured.Results:1.Aspects of Pain improvement:According to the WHO pain evaluation standard,collected from 60 patients with pain,were randomly divided into treatment group 30 cases and control group,30 cases of treatment group complete response in 5 cases,partial in 20 cases,slight easing 3 cases,ineffective in 2 cases,control group complete response in 3 patients,partial in 14 cases,mild ease 8 cases,5 cases failed,with complete response and partial response calculation efficient,pain treatment group effective rate was 83.33%,the control of pain relief rate was 56.66%,the chi-square test,P<0.05,the difference was statistically significant.Combined with the NRS score,the NRS score before treatment in the treatment group was:5.6±1.48 points.The control group of 30 patients with cancer pain was 5.6±1.75 points.By t test,P=1>0.05 showed no statistically significant difference.After treatment,the NRS score in the treatment group was:3.7±2.17 points.In the control group,the score was 3.7±2.17.By t test,the difference of t value P=0.29>was statistically significant at 0.05.2.Quality of life improvement:Patients’ quality of life was assessed based on their scores before and after treatment.In the treatment group,17 cases were improved,8 cases were stable,5 cases were ineffective,and the significant efficiency was 83.33%.In the control group,there were 11 cases of improvement,5 cases of stability,and 14 cases of failure,with an obvious efficiency of 53.33%.The difference was statistically significant by rank-sum test P=0.03<0.05.3.Evaluation of opioid constipation effect:The improvement of constipation was evaluated by pac-sym.In the treatment group,14 cases were cured,12 cases were effective,2 cases were effective,2 cases were ineffective,and the total effective rate was 93.33%.In the control group,9 cases were cured,7 cases were effective,5 cases were effective and 9 cases were not.The total effective rate was 70%.P=0.043<0.05 was statistically significant.4.TCM symptom score:According to the evaluation criteria of cancer pain in traditional Chinese medicine and through the selection of representative symptoms of pain,heat,cold drink,constipation,tongue red or purple,moss yellow or dryness.Before treatment,the P value of>0.05 in terms of TCM symptoms in the treatment group and the control group was statistically significant.After treatment,the scores of TCM symptoms of the two groups decreased compared with that of the former treatment,and the improvement of TCM symptoms of the treatment group was better than that of the control group in terms of the symptoms of heat disturbance,thirst for cold drinks,constipation,tongue red or purple,moss yellow or dryness,and the difference was statistically significant.However,the improvement of fever symptoms was not obvious,and there was no statistical difference in the control group.5.Pain related factors:Peripheral blood pain-related factor endorphins and their changes before and after treatment.The level of peripheral blood endorphins in patients in the two groups before treatment showed no statistically significant difference(P=0.396>,0.05).After treatment,there was a statistically significant difference in peripheral blood levels of endorphin between the two groups,with P=0<0.05.Changes of peripheral blood pain-related factor 5-HT before and after treatment.The level of peripheral blood 5-ht peptide in the two groups before treatment showed no statistically significant difference by P=0.502>0.05.After treatment,the difference in peripheral blood 5-ht level between the two groups was statistically significant with P=0<0.056.Safety evaluation:Safety issues before and during treatment were assessed on the basis of physical and laboratory tests.Our security levels can be divided into four level to level 1 and level 2 calculating safety factor,one of the treatment group the number of level 1 safety for 12 cases,2 levels of security,the number of 14 cases,3 cases of safety,the number of 4 cases,4 security level 0 cases,safety factor is 86.66%,the control group,the number of level 1 safety for 8 cases of 2 levels of safety,the number of 9 cases,3 cases of security number 13 cases,the number of category 4 security 0 cases,safety factor is 56.66%,the test P<0.05,the difference was statistically significantConclusion:Hua Chan Su capsule in treatment of cancer pain can better reduce the pain of patients,improve the patient’s quality of life,part of the TCM symptom scores,relieve patients with constipation,good security,act on the pain of considering patients pain and improve the mechanism of beta endorphin levels,lower levels of 5-HT inflammation index,on clinical has the certain reference
