Investigating the use of Metformin as a prophylactic agent in migraine

Problem: Migraine is a complex, debilitating condition that affects millions of people worldwide. Researchers have long been investigating migraine pathophysiology and its possible associations with other illnesses, such as cardiovascular disease and stroke. There have been recent studies that have investigated the presence of insulin resistance in migraine, an important characteristic which may further our understanding of this disabling illness. The purpose of this review of the literature is to determine if insulin resistance indeed plays a role in migraine, as a better understanding of this complex condition may assist practitioners in more adequate prevention and treatment of these headache attacks. Methods: Full text journal articles in English, published within the last ten years were identified using Pubmed and Ovid Medline searches via Weill Cornell Medical College Library, as well as appropriate references from cited articles. Studies selected for inclusion investigated insulin resistance in migraine. Several studies from this search were also included for background information and discussion. Results: Five studies, which directly investigate the presence of altered insulin metabolism in migraine, were selected for review. Results from each of these studies suggest a positive association between insulin sensitivity or resistance and migraine. Conclusions: Current research suggests a presence of insulin resistance or sensitivity in migraine. Additional studies in larger populations as well as randomized control trials investigating the use of insulin sensitizing drugs in these patients would be beneficial to further establish this association and provide healthcare professionals with more adequate means to prevent and treat this debilitating disease.
