Objective: To preliminary understand the clinical and pathological characteristics of patients with idiopathic membranous nephropathy(IMN)at different ages and guide clinical treatment.Methods: A retrospective analysis was performed at the First Affiliated Hospital of Guangxi Medical University from January 2015 to January 2019.242 patients were diagnosed with IMN and followed up for 12 months.According to age,patients were divided into 62 cases in the elderly group(>60years old)and 180 cases in the young and middle-aged(18-60 years old).Clinical data,laboratory tests and renal pathological results were collected respectively.The results of laboratory examination and renal pathology were statistically analyzed,and the clinical and pathological characteristics of elderly and middle-aged patients were analyzed.Results: 1.The hematuria ratio and cystatin C in the elderly group were higher than those in the young and middle-aged group,P<0.05;the endogenous creatinine clearance,blood urea nitrogen,low-density lipoprotein,and CD8+cell count were lower than those in the young and middle-aged group.,P<0.05.2,the proportion of renal tubular atrophy and interstitial fibrosis in the elderly group is higher than that in the young and middle-aged group,P<0.05.3.The proportion of serum PLA2 R antibody positive in the elderly group is higher than that in the young and middle-aged group.P<0.05.4.At 6 months of treatment,the remission rate of the elderly group was lower than that of the young and middle-aged group,P<0.05;at 12 months of treatment,the remission rate of the elderly group was equivalent to that of the young and middle-aged group,P>0.05.Conclusion: 1.The proportion of hematuria and cystatin C in the elderly group is higher than that in the young and middle-aged group.The endogenous creatinine clearance,blood urea nitrogen,low-density lipoprotein,and CD8+cell count are lower than those in the young and middle-aged group.The count is lower than that of young and middle-aged patients,which may be related to the immune dysfunction between old age and young and middle-aged IMN.2.The proportion of elderly patients with renal tubular atrophy and interstitial fibrosis is more than that of young and middle-aged patients.3.The proportion of serum PLA2 R antibody positive in the elderly group was higher than that of the young and middle-aged.4.The remission rate of elderly IMN patients after 6months of treatment is lower than that of young and middle-aged IMN patients;when treated for 12 months,the remission rate of the elderly group and the young and middle-aged group is equivalent.
