| Objective To explore the clinical and pathological characteristics of immunoglobulin G(Ig G),complement 3(C3)and lymphocyte subsets in patients with idiopathic membranous nephropathy(IMN).Methods Analyze the clinical and pathological data of 65 patients whose renal pathology was confirmed to be IMN under ultrasound guidance from the Department of Nephrology,Chaohu Hospital,Anhui Medical University from December 2012 to December 2020.According to the deposition intensity of Ig G and C3,they were divided into 4 groups,Neg Group,1+ group,2+ group and 3+ group,compare the clinical data and pathological characteristics of each group.Results 1.General situation analysis.A total of 65 patients with IMN were collected from kidney biopsy.The average age is(47.06±15.64)years;the pathological stage is 14(21.54%)in stage 1,43(66.15%)in stage 2,and 8(12.31%)in stage 3.No data on stage 4 cases have been found.Stage 2disease has the highest proportion of all stages.Renal tubule atrophy and interstitial fibrosis 54(83.07%),immunofluorescence Ig G deposition is mainly based on 1,2,and 3intensity,respectively 22(33.84%),21(32.30%),19(29.23%),C3 The deposits are mainly 2 and 3 deposits 14(21.54%)and 44(67.69%).2.Clinical analysis of kidney tissue Ig G,C3 and IMN.The results of this study showed that age,systolic blood pressure,diastolic blood pressure,white blood cells,C-reactive protein,triglycerides,high-density lipoprotein,low-density lipoprotein,immunoglobulin A,immunoglobulin M,complement 3,Complement 4,urea nitrogen,blood creatinine,and glomerular filtration rate were not statistically different(P>0.05);compared with the four groups of albumin,total cholesterol,and 24 h urine protein,the difference was statistically significant(P<0.05));C3 age,systolic blood pressure,diastolic blood pressure,white blood cell,C-reactive protein,triglyceride,high-density lipoprotein,low-density lipoprotein,immunoglobulin G,immunoglobulin A,immunoglobulin M,complement 4.Comparison of urea nitrogen and blood creatinine showed no significant difference(P>0.05);comparison of albumin,total cholesterol,24 h urine protein,and glomerular filtration rate among the four groups showed significant difference(P<0.05)).3.Pathological analysis of kidney tissue Ig G,C3 and IMN.The results of this study showed that there was no statistically significant difference in the comparison of spherical sclerosis,segmental sclerosis,mesangial hyperplasia,and arteriolar wall thickening among the four Ig G groups(P>0.05);comparison of renal tubular atrophy and interstitial fibrosis in the four groups,The difference was statistically significant(P<0.05);there was no statistically significant difference in the comparison of spherical sclerosis,segmental sclerosis,mesangial hyperplasia,and arteriolar wall thickening among the four groups of C3(P>0.05);kidney among the four groups Compared with tubule atrophy and interstitial fibrosis,the difference was statistically significant(P<0.05).4.Analysis of the deposition intensity of serum Ig G,C3 and kidney tissue Ig G,C3 in IMN.The results of this study show that the concentration of serum Ig G has nothing to do with the intensity of Ig G deposition in kidney tissue(P>0.05).The stronger the C3 deposition intensity in kidney tissue,the lower the C3 level in serum(P<0.05).5.Analysis of lymphocyte subsets and IMN.The results of this study showed that there was no statistically significant difference in CD4 +,CD8 +,CD4+ /CD8+ between the four Ig G groups(P>0.05);the difference in CD8 + between the four groups of C3 was not statistically significant(P>0.05);CD4+Compared with CD4+/CD8+,the difference was statistically significant(P<0.05).6.The relationship between immunoglobulin,complement and lymphocyte subsets in different pathological stages.The results of this study showed that with the increase of pathological stages,there was no statistically significant difference in Ig G,Ig A,Ig M,C4,CD4+,CD8+,CD4+/CD8+(P>0.05);C3,the difference was statistically significant(P<0.05).Conclusions In the pathological stages of patients with idiopathic membranous nephropathy,stage 2 lesions have the highest proportion of all stages,renal tubular atrophy and interstitial fibrosis 54(83.07%);IMN patients with the increase in the intensity of immunofluorescence Ig G and C3 deposition,serum Albumin level decreased,total cholesterol increased,24 h urine protein level increased,renal tubular atrophy and interstitial fibrosis were obvious;with the increase of the deposition intensity of renal tissue C3,CD4+,CD4+/CD8+ showed an increasing trend.In summary,the degree of Ig G,C3 deposition in kidney tissue and the levels of Ig G,C3 and lymphocyte subsets in serum are closely related to IMN,indicating that the examination of the above indicators is of great significance for the early prediction of IMN and the judgment of the severity of the disease. |
