ObjectiveThis study investigated the clinical effects of Qi Tian granule recipe on postoperative inf lammation of elderly hip fracture patients by observing the effects of Qi Tian granule recipe on postoperative inflammation of elderly hip fracture patients,and provided a reference for regulating the inflammatory response in elderly patients with hip fractures.MethodThis study adopted a prospective study to select elderly unilateral hip fracture patients(including femoral neck fractures and femoral intertrochanteric fractures)admitted to our hospital from May 2019 to February 2020,and patients undergoing unilateral artificial femoral head replacement or PFNA internal fixation during hospitalization,were randomly divided into Qi Tian granule group and control group according to the inclusion and exclusion criteria.A total of 60 cases were included,of which 30 were in the Qitian granule group and 30 were in the control group.Control group:Cefazolin sodium 2g+0.9%normal saline 100ml,q12h,continuous use for 2 days.The anticoagulation regimen was subcutaneously injected with nadroparin calcium injection 12 hours after surgery,once a day during hospitalization.The Qi Tian Granules group was treated with Qi Tian granule recipe for 7 days on the basis of the control group.Record the WBC,neutrophil count,CRP,IL-6,TNF-α,TCM symptom score,TCM syndrome effect before,on the first,third,and seventh days after surgery.SPSS 23.0 statistical software was used to analyze the differences of each index between Qi Tian granule group and control group.Result1.In terms of baseline data,there were no statistically significant differences in gender,age,fracture site,preoperative complications,Operation time and intraoperative blood loss(P>0.05);There were no significant differences in white blood cell count(WBC),neutrophil count,C-reactive protein(CRP),Interleukin 6(IL-6),and Tumor necrosis factor(TNF-α)(P>0.05);The two groups were comparable.2.Repeated measurement analysis of variance and multivariate analysis of variance were performed on WBC,neutrophil count,CRP,IL-6,TNF-α levels at different time points in the two groups,and the results suggest that time has an effect on the changes of each index in the two groups(P<0.05).There was no significant difference in WBC,neutrophil count,CRP,IL-6,TNF-αlevels between the two groups on the first day after operation(P>0.05).There were no significant differences in neutrophil counts,CRP,IL-6,and TNF-αlevels between the two groups on the third day after operation(P>0.05).There were no significant differences in WBC,neutrophil counts,and CRP levels on the seventh day after operation(P>0.05).Compared with IL-6 and TNF-α levels on the seventh day after surgery,the Qitian granule group was lower than the control group,and the differences were statistically significant(P<0.05).3.In terms of TCM syndrome score and efficacy:The total effective rate of QiTian Granule Group was 76.67%,and the total effective rate of Control Group was 36.67%.In terms of overall efficacy,QiTian Granule Group was superior to the control group(P<0.05);Compared with before treatment,the TCM syndrome scores of the two groups after treatment were all lower than before,and there was a statisticant difference(P<0.05).The QiTian granule group was better than the control group in improving the TCM syndrome score(P<0.05).4.The subjects of the two groups did not experience any discomfort during the experimental period,the vital signs of the two groups did not show significant fluctuations,and neither group reported any adverse reactions such as gastrointestinal reactions or systemic reactions.ConclusionApplying QiTian granules after surgery can effectively regulate the postoperative inflammation of hip fractures in the elderly,and improve the symptoms of Qi deficiency and blood stasis in elderly hip fracture patients.
