The Clinical Research Of Blood Hypercoagulable State In Hip Fracture Of Aged People With Qi Tian Granule

Objective:To explore the influence of self-prepared decoction of Qi Tian granule(decoction)on hypercoagulable state after hip fracture,for providing the treatment of clinical Blood Hypercoagulable State a scientific basis,at the time of clinical observation,the final intervention effect was evaluated,and the clinical results were obtained with reference to the analysis results.We compare the D-Dimer,APTT,PT,Fib,TT and CDus of deep vein before and after operation to accomplish this objective.Methods:From 1,2018 to 6,2019,all resident patients of hip fracture(including femoral neck fracture,intertrochanteric fracture)in the examination iscarried out at the time of the initial admission of the elderly.And all of then treated for the PFNA or hemiarthroplasty in elderly patient included with TCM syndrome of qi deficiency and blood stasis syndrome,Then 60 patients were screened out and randomly divided into two groups,including the treatment group(n=30)and the control group(n=30).The treatment group was demanded to take with Qi Tian granule(decoction)and subcutaneous injection the medicine of low molecular weight heparin(LMWH),and the control group only given subcutaneous injection of low molecular weight heparin(LMWH)between before operation day 1 to Day 6 after operation.Both group received routine anti infection.We observe the blood data of D-Dimer,PT,APTT,Fib and TT between before operation day 1 and Day 1,3,7 after operation.The affected lower extremity vascular color Doppler examination were also needed in before operation day 1 and Day 7 after operation.the incidence of DVT and occurrence of side effects were observedResulisThere was no significant difference in D-Dimer,Fib,PT,APTT,TT between the two groups before operation(P>0.05)After operation,the levels of D-Dimer,Fib were lower,and the changes in the treatment group were more significantly than those in the control groups.The difference in the levels of D-Dimer on the Day 3,7 after operation were statistically significant(P<0.001).The difference in the levels of Fib on the Day 3,7 after operation were statistically significant(P<0.05).The levels of PT,APTT were significant higher in the combined traditional Chinese Qi Tian granule and western medicine group than the LMWH group(P<0.05);The difference in the levels of PT,APTT on the Day 7 after operation were statistically significant(P<0.05).No side effect and complication were found in both groups.ConclusionBased on the current clinical evidence,the combination of Qi Tian granule and low molecular heparin can effectively interfere with coagulation and fibrinolysis indicators in patients and has a faster therapeutic effect,its salient feature are rapid treatment of blood hypercoagulable state post major orthopedic surgery.it is worthy of popularization and application.
