The report of the 18 th national congress of the communist party of China clearly points out that the goal of equal access to basic medical and health services is not only conducive to promoting balanced regional development,but also an important prerequisite for building a moderately prosperous society in all respects.This paper calculates the level of basic medical and health services and their equalization degree in each county of Sichuan province,analyzes the factors affecting their equalization development,and puts forward some Suggestions to promote the equalization development of basic medical and health services in Sichuan province.The main contents of the paper include the following aspects:Firstly,the paper reviews the literature from the perspective of equalization measure,influencing factors and countermeasures of basic medical and health services,defines the related concepts of equalization of basic medical and health services,and reviews the four basic theories of equalization,namely,the theory of public goods,the theory of fiscal decentralization,the theory of equity and justice,the theory of new public management and the theory of service,providing theoretical support for the follow-up research.At the same time,the development status of basic medical and health services in cities and prefectures in Sichuan province is described.Secondly,the paper constructs an evaluation system of basic medical service development composed of 3 first-level indicators and 9 second-level indicators,and uses factor analysis method to determine the weight of each indicator.Then the overall development level and sub-development level of basic medical and health services in each county of Sichuan province were calculated.The following conclusions are drawn: first,in general,compared with other counties,the development level of basic medical and health services in municipal districts and county-level cities is generally higher;Second,in terms of regions,the development level of basic medical and health services in the eastern border of Sichuan province and some districts and counties of Liangshan Yi autonomous prefecture is low.Thirdly,the paper measures the equalization degree of the basic medical and health services in each district and county from the overall and sub-item aspects,and uses cluster analysis to divide the cities and prefectures in Sichuan into three regions and the counties in Sichuan into four regions,and makes corresponding analysis.The conclusions are as follows: first,generally,the equalization level of all districts and counties in Sichuan province has been improved,but it still presents a non-equal pattern;Second,in terms of sub-indicators,the gap between counties in the setting of medical institutions and the allocation of medical and health resources has been reduced,while the gap in the utilization of medical and health services has been relatively enlarged.Thirdly,whether the cities of Sichuan province are divided into three categories or four categories according to the county region,there is an obvious gap in the level of basic medical and health services.Fourthly,the panel model is established to analyze the factors affecting the equalization degree of the basic medical and health services in each district and county in Sichuan province.The conclusions are as follows: fiscal expenditure decentralization,per capita medical and health expenditure,and per capita GDP all have significant positive effects on it,and according to the size of contribution,it is per capita GDP,fiscal expenditure decentralization and per capita medical and health expenditure.Finally,based on the results of empirical research,some suggestions are put forward to promote the equalization of basic medical and health services in Sichuan province : the financial autonomy of local governments should be expanded reasonably;actively developing local economy;increase government spending on medical and health care and optimize the structure of government spending;vigorously promote openness in medical and health affairs.
