| Objectives:The purpose of this survey is to understand the iodine status and prevalence of thyroid disorders among adults in Yanji,and explore the risk factors of thyroid disorders.Methods:This is a cross-sectional study.From October to December 2014,We use stratified cluster random sampling to select residents over the age of 18 and have lived in Yanji for more than five years.We conducted a questionnaire,physical examination,Laboratory examination and thyroid Doppler ultrasound examination on the selected residents.The data were analyzed and processed by SPSS 25.0.We calculated the median urinary iodine and the prevalence of thyroid disorders among residents.And compared the differences of the median urinary iodine and the prevalence of subclinical hypothyroidism and thyroid nodules of residents in each group by nonparametric test,chi-square test or Fisher’s exact test.Finally,we analyzed the risk factors of subclinical hypothyroidism and thyroid nodules by logistic regression analysis.Results:(1)The median urinary iodine of adults in Yanji was 194.70 μg/L.And the median urinary iodine of residents with different age,different eating habits of kelp or laver,and different lipid metabolism indexs were statistically significant(P < 0.05).(2)The prevalence of thyroid disorders of adults in Yanji was as follows:subclinical hypothyroidism was 0.1%,subclinical hyperthyroidism was 0.1%,hypothyroidism was 0.9%,subclinical hypothyroidism was 12.7%,thyroid nodules was 23.4%,goiter was 2.4%,Tg Ab negative was 11.7%,TPOAb negative was 11.0%.(3)An older age,a higher BMI and TPOAb positive were risk factors for thyroid nodules.Compared with young people,the prevalence of thyroid nodules in middle-aged and elderly people was higher(OR=1.955,95%CI:1.356-2.819;OR=2.586,95%CI:1.247-5.362).Compared with normal weight people,the prevalence of thyroid nodules in overweight and obese people was higher(OR=1.523,95%CI:1.061-2.185;OR=2.359,95%CI:1.423-3.910).Compared with TPOAb negative people,the prevalence of thyroid nodules in TPOAb positive people was higher(OR=1.753,95%CI:1.097-2.800).(4)An older age,TPOAb positive and Tg Ab positive were risk factors for subclinical hypothyroidism.Compared with young people,the prevalence of subclinical hypothyroidism in elderly people was higher(OR=3.178,95%CI:1.432-7.053).Compared with TPOAb negative people,the prevalence of subclinical hypothyroidism in TPOAb positive people was higher(OR=1.918,95%CI:1.059-3.474).Compared with Tg Ab negative people,the prevalence of subclinical hypothyroidism in Tg Ab positive people was higher(OR=1.958,95%CI:1.095-3.501).Conclusions:(1)The iodine status of adults in Yanji was suitable.(2)The prevalence of thyroid disorders of adults in Yanji was similar to that in the whole country.Including hyperthyroidism,subclinical hyperthyroidism,hypothyroidism,subclinical hypothyroidism,thyroid nodules and goiter.(3)An older age,a higher BMI and TPOAb positive were risk factors for thyroid nodules.(4)An older age,TPOAb positive and Tg Ab positive were risk factors for subclinical hypothyroidism. |
