| Objective: To analyze and evaluate the functional outcome of shoulder joint function recovery by using modified in situ suture technique to treat Ellman gradeⅢ supraspinatus tendon tear.Methods: According to the inclusion and exclusion criteria,42 patients with Ellman Grade Ⅲ supraspinatus tendon joint tears treated by shoulder Arthroscopy in our hospital(Yanbian University Hospital)from January 2017 to January 2020 were selected,including 22 patients in group An and 20 patients in group B with modified in situ suture technique.VAS score was used to evaluate the pain before and after operation(2 weeks,1 month,3 months,6 months and 12 months).ASES、CSS and UCLA scores were used to evaluate the effect of two kinds of suture techniques on the functional recovery of shoulder joint in each period after operation.Results: A total of 42 patients were followed up for more than 12 months.The VAS scores of patients in gro up A were(7.05±1.21),(6.95±0.90),(6.36±0.90),(4.36±0.73),(2.59±0.80),(1.00±0.69)respectively before operation,2weeks,1 month,3 months,6 months and 1 year after operation.The VAS scores of patients in group B were(6.95±1.32),(6.10±0.98),(5.40±0.99),(3.50±0.83),(2.25±0.79)and(0.85±0.67)respectively before operation,2 weeks,1 month,3months,6 months and 1 year after operation.The ASES scores of patients in group A were(37.64±10.03),(34.50±6.49),(45.36±6.76),(61.27±6.33),(79.23±4.99),(92.45±3.52)respectively before operation,2 weeks,1 month,3 months,6 months and12 months after operation.The ASES scores of patients in group B were(34.20±5.41),(41.40±6.39),(54.20±6.32),(66.35±5.28),(78.70±5.66),(93.20±3.49)respectively before operation,2 weeks,1 month,3 months,6 months and 12 months after operation.The CSS scores of patients in group A were(40.68±5.67),(40.55±6.13),(50.95±5.20),(62.91±4.60),(75.77±5.12),(86.41±2.56)respecttively before,2 weeks,1 month,3 months,6 months and 12 months after operation.The CSS scores of patients in group B were(40.30±5.38),(46.35±4.48),(55.30±4.63),(66.95±4.27),(77.20±3.86),(86.30±2.85)respectively before operation,2 weeks,1 month,3 months,6 months and 12 months after operation.There were significant differences in VAS score,ASES score and CSS score between the two groups at 2 weeks,1 month and 3 months after operation.There was no significant difference in operation time between the two groups(P>0.05).There was significant difference between the patients in the same group before operation and at the last follow-up(P<0.05),but there was no significant difference in the curative effect between the two groups at the last follow-up(P>0.05).In group A,the results were excellent in 6 cases,good in 13 cases and fair in 3 cases.The excellent and good rate was 86.36% in group B,excellent in 7 cases,good in 12 cases and fair in 1 case,with an excellent and good rate of 95.0%.Conclusion: In the treatment of lateral tear of supraspinatus tendon under Arthroscopy,the early postoperative effect of the modified in situ suture group was better than that of the expanded suture group,and the operation was simple and could achieve the effect of "borrowing force to reduce tension".The healing time of supraspinatus tendon is shorter,the rehabilitation period of shoulder joint is shorter,and the function recovery is faster,which is worthy of clinical promotion. |
