| Objective: In this study,by recording and analyzing the block status and basic monitoring of supraclavicular brachial plexus block in different groups, the effect of nalbuphine and dexmedetomidine combined with local anesthetics on supraclavicular brachial plexus block was explored. Methods: From October 2019 to August 2020,90 patients with upper limb fracture internal fixation under the supraclavicular brachial plexus block under the guidance of ultrasound in Wuhu Second People’s Hospital were selected. The subjects were randomly divided into three groups.Group C was given 0.5% ropivacaine +0.9% normal saline;G1group was given 0.5% ropivacaine+10mg nalbuphine;G2 group was given 0.5%ropivacaine+0.75μg/kg dexmedetomidine;the total dose of each group was 20 ml.Record the onset time of sensory and motor block, the duration of block, the duration of analgesia, changes in basic vital signs, and the number of cases of adverse reactions in each group, and compare the differences in monitoring indicators between each group. Results: Here was no statistically significant difference between the three groups of patients in basic data such as weight and age(P>0.05);G1 and G2 groups can effectively accelerate the onset of sensory and motor block, prolong sensory,The duration of motor block and the duration of postoperative analgesia were significantly different(P<0.05);and the G1 group was better than the G2 group, and the difference was statistically significant(P<0.05);The three groups were in poor condition In the comparison of responses, the incidence of sleepiness in the G2 group was significantly higher than that of the other two groups, and the difference was statistically significant(P<0.05).There was no statistically significant difference among the three groups of changes in heart rate(HR)and mean arterial pressure(MAP)(P>0.05),and the differences within the three groups of HR and MAP changes were statistically significant(P< 0.05),the G2 group had the most significant changes in MAP.Conclusion: Compared with the simple ropivacaine group, the combination of nalbuphine or dexmedetomidine can improve the blocking effect and prolong the postoperative analgesia time.Compared with dexmedetomidine, nalbuphine is effective in analgesia. The action lasts longer and causes less adverse reactions. |
