Research On Chronic Disease Management Mode Of The Integration Between Treatment And Prevention Under The Background Of Medical Alliance

Objective Based on the path perspective of "medical prevention integration",this study carried out different focuses on the typical chronic disease management mode under the background of medical confederacy in China.In theory,the common logic relationship of chronic disease management realization path under the background of domestic medical confederacy was summarized.Empirical on couplet of guangdong province based on the medical conduct based on "medical proof fusion" slow disease management cases respectively in breadth and depth of focus on analysis,aims to explore how to play the medical couplet body and the advantage of "medical proof fusion" path,improve slow disease management effect,for our country to explore grassroots medical prevention by fusion "slow disease management model provides reference basis.Methods Data were collected by literature and case study.PEST analysis is used to identify the key influencing factors of chronic disease management in the context of medical confederacy.Combined with explanatory structure model method,the relationship between influencing factors is identified,the logical commonness of influencing factors is expounded,and the common explanatory structure model of chronic disease management in the context of medical confederacy is constructed.From the perspective of "integration of medical treatment and prevention",the construction of chronic disease management under the background of medical association in Guangdong Province was analyzed,and the outcome of hypertension patients in medical and health institutions in Guangdong Province was analyzed by dynamic series analysis to evaluate the construction effect.From the perspective of "medical integration and prevention integration",the chronic disease management mode in a county in western Guangdong was analyzed.Based on the pilot data of medical reform on chronic disease management at the county level,the baseline situation of the observation group and the control group and the situation after 1 year of follow-up were investigated and recorded,and semi-structured interviews were conducted to evaluate the effect of the mode.Use the situation analysis method,carry on the conclusion analysis,make the policy suggestion.Results The explanatory structure model of the influencing factors of chronic disease management was constructed in the context of medical association.It was believed that there were five levels of logical progressive correlation among the factors.Through the policy environment,the economic environment was determined,the technological environment was driven,and finally the social environment was influenced,and the effect of chronic disease management was directly determined.In the case of Guangdong Province,the construction of chronic disease management based on "integration of medical treatment and prevention" has a good policy environment,which can improve the situation of hierarchical diagnosis and treatment and guide patients with chronic diseases to return to the grassroots level.However,the balance of medical resource allocation is weak,and the grassroots level has a strong need to explore "integration of medical treatment and prevention".In the case of a county in western Guangdong,the chronic disease management mode based on"integration of medical treatment and prevention" can rely on the management advantages of the compact county medical community to carry out chronic disease management services,promote the sinking of resources,improve the service capacity of the grassroots,and show strong dependence on the government,which needs to focus on solving the problems related to the social environment.Conclusions The advantages of the chronic disease management mode based on"medical integration" in the context of medical association are as follows:to construct the chronic disease management mode with the idea of "medical integration",effectively utilize the resources of medical association,and improve the ability of chronic disease management;The disadvantage is that it has certain requirements on the resource allocation and technical level of the member units.The opportunities are:the need for innovation of chronic disease management mechanism is strong,and the government attaches great importance to the exploration and practice of chronic disease management;The challenge is that local policies and systems may not be suitable for model construction.According to the situation analysis,policy suggestions were given.Medical workers should take the "integration of medical treatment and prevention" as the path,and actively strengthen the exploration of chronic disease management mode based on the advantages of policy effect.Medical confederations should carry out reform according to local conditions,make full use of existing conditions,and promote slow reform;Policy makers should improve the construction of institutions and systems to provide a guarantee for the exploration of reform;The government should provide appropriate policy support and financial subsidies.
