Effect Of Different Perioperative Time Of Acupuncture Intervention On Postoperative Urination Function In Patients Undergoing Mixed Hemorrhoid Operation

Abstract/Summary:
ObjectivePostoperative dysuria is the most common complication in anorectal department.Studies have shown that acupuncture can relieve postoperative dysuria and improve patients’postoperative comfort.However,there is no unified understanding on when acupuncture has the best therapeutic effect.In this study,by comparing acupuncture at different time points,the best time for acupuncture to prevent dysuria after mixed hemorrhoids operation was discussed,so as to provide a basis for clinicians to make decisions.MethodsIn this prospective randomized controlled trial design,152 subjects were divided into group A(preoperative acupuncture),group B(intraoperative acupuncture),group C(postoperative acupuncture),and group D(observation group)by random number table method,with 38 patients in each group.The three groups A B and C were treated with acupuncture intervention 30 minutes before operation,during operation(after anesthesia)and 6 hours after operation.The two sides of Zhong liao(BL 33)and Hui yang(BL35)were selected,and 2.5 inch were punctured and electroacupuncture(50Hz)was applied for 1520min.The observation group received no acupuncture treatment.The differences in the length of time from the end of surgery to the first urination,the volume of the first urination,the perception score of voiding patency,the pain score(VAS),the incidence of urinary retention,and the rate of catheterization were observed between the different timing of acupuncture interventions.All data were managed in SPSS 26.0,and statistical analysis was carried out by variance analysis,x2 test,rank sum test.The test level was α=0.05(bilateral).ResultsThe final 141 patients were included,Group A(34 cases),Group B(36 cases),Group C(35 cases),and Group D(36 cases),and 11 cases were decompensated.The results showed that there was no significant difference in age,sex,height,weight,anesthetist,surgical,anesthesia mode,operation duration,blood loss and infusion amount in the four groups.The median time intervals from the end of the operation to the first urination in Group A,Group B,Group C,and Group D were 12.765,13.265,9.330,and 15.195,respectively.Compared with the other three groups,the interval in group C was shorter,and the difference was statistically significant(P=0.007,0.049,0.005<0.05).The median postoperative urine volume of the first time in group A,B,C and D was 330.00,340.00,320.00 and 220.00,respectively.There was statistically significant difference in the first urination volume between the four groups(Kruskal-Wallis x2=9.342,P=0.025<0.05).After multiple comparison,the difference between group C and group D was statistically significant(P=0.039<0.05).Compared with group D,there was statistically significant difference in the score of patency of urination in groups A,B and C(P=0.049,0.017,0.003<0.05),but there was no statistically significant difference among the three groups.Group C had the lowest incidence of urinary retention(8.71%),but there was no statistically significant difference in the incidence of urinary retention among the four groups(P=0.453>0.05).There was no significant difference in the rate of catheterization between the four groups(P=0.882>0.05).The postoperative pain score(VAS)of the four groups at 8h was statistically significant(P=0.008<0.05),group C was superior to group B(P=0.021<0.05),and group C was superior to group D(P=0.016<0.05).ConclusionPostoperative acupuncture intervention can shorten the time interval from the end of the operation to the first urination,improve the patient’s postoperative urination experience,and relieve postoperative pain.
