| Objective:Guided by the related theories about traditional Chinese and Western Medicine,to analyze the etiology and pathogenesis of PONV(postoperative nausea and vomiting)and its managerial principle,to observe the clinical effect of Small Pinelliae Decoction on the prevention and treatment of nausea and vomiting caused by Syndrome of Retention of Phlegm after general anesthesia for benign bone tumor of limbs,and to provide a clinical basis for prevention and treatment of PONV of traditional Chinese medicine.Methods:Select 80 patients with osteoid osteoma,osteochondroma,enchondroma,bone cyst and other benign bone tumors by diagnosing tentatively from Jan.2020 to Dec.2020 who met the inclusion criteria,and randomly divide them into treatment group and control group with 40 patients in each group.Patients of each groups were given a unified anesthesia plan and analgesia pump.The treatment group was given one dose of Small Pinelliae Decoction,a Chinese medicine granule,in the morning and evening of the day before operation and in the morning of the second day after operation,while the control group was given the same amount of placebo in the morning and evening of the day before operation in the evening of operation and in the morning of the second day after operation.Observe and record the two groups on the occurrence of PONV cases,severity,duration,and additional metoclopramide cases among 0-4 h,4-12 h,12-24 h after postoperative,in addition to TCM Symptom Score at PONV occurring and postoperated 24 h.It calculates the total incidence and the effective rate of PONV in the two groups,and analyze the function of Small Pinelliae Decoction on the frequency,severity and duration of PONV.Results:There are 10 cases were eliminated and 70 cases finished successfully in the study.Each group comprises 35 patients.There was no significant difference in baseline data between the two groups(P > 0.05).The control groups can compare with the treatment group.After intervention,the incidence of PONV in the two groups within 24 h after operation was: 10cases(28.6%)in the treatment group and 32 cases(91.4%)in the control group.The incidence and total incidence in the treatment group were lower than those in the control group at three periods,and the differences were statistically significant(P < 0.05).PONV severity were classification comparison: In 0 to 4 h period of time,the cases of Ⅰ,Ⅱ,Ⅲ in treatment group were fewer than that in the control group;In 4 to 12 h period of time,the cases of Ⅰ,Ⅱ,Ⅲ in treatment group were fewer than that in the control group;In 12 to 24 h period,no patients in both groups performed Ⅱ symptoms,the cases of Ⅰ,Ⅲin treatment group were fewer than that in the control group;The difference was statistically significant(P < 0.05).The duration of PONV and the number of patients receiving metoclopramide were lower in the treatment group than in the control group,with statistical significance(P < 0.05).In terms of efficacy,there was no statistically significant difference in TCM syndrome scores between the two groups before treatment(P > 0.05),and the difference in scores between the two groups after treatment was statistically significant(P < 0.05).The difference in the difference value of scores between the two groups was statistically significant(P < 0.05).The difference of the effective rate between the two groups was statistically significant(P < 0.05).The effective rate of the treatment group was higher than that of the control group.Conclusion:Small Pinelliae Decoction can reduce the incidence of nausea and vomiting after general anesthesia for benign bone tumor of extremities,reduce the grade of severity,and shorten the duration of postoperative nausea and vomiting.It shows a certain effect on the prevention and treatment of PONV,and provides a clinical basis for the prevention and treatment of PONV with traditional Chinese medicine. |
