Objective To explore how ultrasound elastography and superb microvascular imaging(SMI)combined with the 2017 American College of Radiology(ACR)thyroid imaging reporting and data system(TI-RADS)can distinguish benign and malignant thyroid nodules.Analyze the diagnostic value of ultrasound elastography,SMI,ACR TI-RADS classification criteria and the combined application of the three in the assessment of benign and malignant thyroid nodules.Methods A total of 156 thyroid nodules in 141 patients admitted to our hospital from December 2018 to December 2020 were selected as the research objects,All nodules are classified according to the ACR TI-RADS classification criteria,and ultrasound elastography and SMI examinations are performed before surgery.The elasticity score in ultrasound elastography examination is greater than 3 points,the blood flow distribution pattern in SMI is low blood supply or peripheral blood flow combined with perforating blood flow signals and TR ≥ 4 in the ACR TI-RADS classification criteria is judged to be malignant nodules,and all nodules are based on the pathological results obtained from surgery or needle biopsy as the gold standard.Analyze the diagnostic value of ultrasound elastography score,SMI blood flow distribution pattern,ACR TI-RADS classification criteria and the combination of the three in differentiating benign and malignant thyroid nodules.Results There are 156 thyroid nodules,97 benign nodules and 59 malignant nodules.The sensitivity of the combined diagnosis of thyroid nodules by ultrasound elastography and SMI combined with ACR TI-RADS classification criteria was 81.4%,specificity was88.1%,accuracy was 84%,positive predictive value was 91.9%,and negative predictive value was 74.3%.Compared with ultrasonic elastography alone,the diagnostic efficiency of ultrasound elastography combined with SMI and ACR TI-RADS classification criteria in the diagnosis of thyroid nodules was significantly improved,and the difference was statistically significant(P <0.05).Conclusion Ultrasound elastography and SMI combined with ACR TI-RADS classification criteria have good diagnostic power in the assessment of benign and malignant thyroid nodules.
