Objective: To observe the clinical efficacy of injury capsule combined with percutaneous kyphoplasty of vertebral body(PKP)on patients with osteoporotic vertebral compression fracture(OVCF)of kidney deficiency and blood stasis type,and analyze the effects of the treatment methods on bone resorption marker β-CTX and bone formation marker P1 NP in OVCF patients,so as to provide theoretical basis for the prevention and treatment of OVCF with injury capsule and traditional Chinese medicine.Methods: A total of 60 OVCF patients with kidney deficiency and blood stasis admitted in the Department of Spinal minimally invasive Orthopaedics,Gansu Provincial Hospital of traditional Chinese Medicine from January 2019 to June 2020 were randomly divided into observation group(n = 30)and control group(n = 30).Two groups of patients after admission to perfect the relevant examination were performed after PKP holds the operation,patients in the control group were treated with calcium carbonate D3 granule and etodecalcitonin after PKP operation,observation group on the basis in the control group treatment add damage capsule treatment,two groups of patients were treated 12 w.Surgery in patients with postoperative record,respectively in the preoperative and postoperative 3 d,12 w record and compare the two groups patients postoperative pain visual analogue scale(VAS score),record and compare the last follow-up clinical curative effect of two groups patients.On the morning before PKP surgery and the next day after treatment,venous blood was collected under fasting state and serum was extracted.The expression levels of β-CTX and P1 NP,bone transition indexes,were detected and compared between the two groups before and after treatment by ELISA.SPSS24.0 statistical software was used to analyze the observed data.Results:1.There was no difference in the general clinical data(including gender,age,height,weight,etc.)between the two groups(P > 0.05),indicating comparability.2.Comparison of VAS scores at preoperative and postoperative time cut-off points between the two groups: intra-group comparison: VAS scores at 3 days after surgery,12 w after surgery compared with that before surgery,and 12 w after surgery compared with that after 3 days after surgery were significantly decreased(P < 0.05);Comparison between groups: There was no difference in VAS scores between the two groups before and 3 days after surgery(P >0.05).The VAS score of the observation group was significantly lower than that of the control group at 12 w after surgery(P < 0.05),indicating that the long-term effect of the observation group on the relief of lumbar and back pain was better than that of the control group.3.Bone resorption marker β-CTX :showed no difference in serum β-CTX content between the two groups before treatment(P =0.718 > 0.05).The serum β-CTX content of subjects in both groups was significantly decreased 12 weeks after treatment compared with that before treatment(P < 0.05);After 12 weeks of treatment,the serum β-CTX content of the observation group was significantly lower than that of the control group(P=0.004 < 0.05),indicating that the treatment of the observation group was better than the control group in reducing the serum β-CTX expression level of the patients.4.Bone morphogenetic marker P1NP: there was no difference in P1 NP serum content between the two groups before treatment(P =0.760 > 0.05).Comparison between the two groups: P1 NP serum content of patients in both groups after 12 weeks of treatment was lower than that before treatment(P < 0.05);However,after 12 weeks of treatment,there was no difference in serum P1 NP content between the two groups(P= 0.534> 0.05).5.Bone mineral density: There was no significant difference between the two groups of subjects before treatment in BMD(P=0.501>0.05);after 12 weeks of treatment,the observation group had a significantly higher BMD than the control group,and the difference was statistically significant(P=0.001<0.05).Comparison within the group: The bone mineral density of the two groups of patients was increased after 12 weeks of treatment compared with before treatment(P<0.05).6.Comparison of clinical efficacy during the last follow-up: the two groups of patients underwent the final clinical efficacy evaluation at 12 weeks of treatment,the total effective rate of the observation group(96.67%)> the total effective rate of the control group(80.00%),there was no difference between the two groups(P < 0.05).Conclusion: After PKP surgery for OVCF patients with kidney deficiency and blood stasis type,injury capsule intervention treatment can significantly alleviate the symptoms of patients with back pain,Increase the bone mineral density,with significant clinical efficacy,and can significantly reduce the expression level of serum β-CTX,thus inhibiting bone absorption,anti-osteoporosis,which is safe and effective and worthy of clinical promotion and application.
