Professor Liu Baohou’s Experience In The Diagnosis And Treatment Of Primary Nephrotic Syndrome With Integrated Traditional Chinese And Western Medicine

Objective: To summarize Professor Liu Baohou’s clinical experience in the diagnosis and treatment of primary nephrotic syndrome with integrated traditional Chinese and western medicine,analyze the rule of medication,and inherit his academic thought,so as to better guide the diagnosis and treatment of patients with primary nephrotic syndrome.Methods:(1)Follow up learning: learn and summarize Professor Liu’s academic thoughts and clinical experience in the diagnosis and treatment of primary nephrotic syndrome by means of clinical analysis,follow up and copy prescription,case discussion and other methods.(2)Literature research: refer to relevant literatures and books,learn and summarize Professor Liu Baohou’s understanding of the etiology and pathogenesis of primary nephrotic syndrome,and summarize the key points of integrated traditional Chinese and western medicine in the treatment of primary nephrotic syndrome.(3)Data mining: collected in January 2019 to June 2020 in gansu university of traditional Chinese medicine hospital professor Liu Baohou outpatient prescription data of all patients with primary nephrotic syndrome in data mining,prescription and medication,finishing professor liu combine traditional Chinese and western medicine diagnosis and treatment of primary nephrotic syndrome academic thoughts.Results:(1)Professor Liu Baohou believed that the basic pathogenesis of primary nephrotic syndrome was the deficiency of the root and the deficiency of the lung,spleen,liver and kidney,especially the deficiency of the spleen and kidney,and the deficiency of wind,dampness and heat and blood stasis.(2)Professor Liu Baohou has put forward important academic views on the diagnosis and treatment of primary nephrotic syndrome,such as "dampness and heat cannot be removed,protein is difficult to remove","blood stasis cannot be removed,kidney qi is difficult to recover".In terms of treatment,it is advocated to clear dampness-heat,promote blood circulation and remove blood stasis,treat from wind,and often take the use of hormone as the key point of integrated traditional Chinese and western medicine.(3)The application of the inheritance of traditional Chinese medicine auxiliary platform V2.5"software for the entry of 200 found the first prescription for data mining,the use of traditional Chinese medicine(TCM)142,which USES frequency of 10 or more drugs,48,drug use frequency of the top ten is in turn: astragalus,angelica,earthworm,leech,pyrrosia leaf,fructus ligustri lucidi,radix rehmanniae,motherwort,Fried atractylodes,wind;The statistical results of four qi frequency showed that Professor Liu mainly used cold,warm and flat drugs.The statistical results of five-frequency taste frequency showed that the main drugs were sweet,bitter and acorn.The statistics of drug channelling showed that more drugs were channelling into liver,spleen,lung and kidney.Based on association rule analysis,45 drug pairs with high frequency of use were obtained,among which the top five drug pairs were:Leech-Dilong,Astragalus-Angelica,Astragalus-Dilong,Leech-Astragalus,Leech-AstragalusDilong;Based on entropy clustering algorithm,36 drug pairs,18 core combinations and 9new prescriptions with high correlation degree were obtained.Conclusion:(1)Professor Liu Baohou advocated the "dual diagnosis of Chinese and Western medicine and the organic combination of Chinese and Western medicine" in the diagnosis and treatment of primary nephrotic syndrome.Pay attention to dispel evil and change the force contrast between the two sides of evil and positive,to achieve the effect of "dispel evil and positive and self-safety".In treatment,different stages of hormone use are often used as the combination point of TCM and Western medicine for syndrome differentiation and treatment,with distinctive diagnosis and treatment characteristics.(2)Through data mining technology objectively demonstrated that professor Liu Baohou treating primary nephrotic syndrome pay attention to the hot and humid,promoting blood circulation to remove blood stasis,both inside and outside the wind and etc.,the result accord with professor liu "damp and hot is not in addition to,protein difficult to disappear","blood stasis,kidney difficult complex","from the wind differentiation",such as academic point of view,has a certain clinical application and popularization value.
