Polycystic Ovary Syndrome(PCOS)is a common gynecological endocrine disease in women og adolescent and reproductive age,PCOS is characterized by clinical and/ or biochemical signs of hyperandrogenism,oligo-and/or anovulation,and polycystic ovaries on ultrasound examination,and is often associated with abnormal glucose and lipid metabolism.Current studies show that PCOS patients are more likely than normal women to have psychiatric disorders,especially the prevalence of depression,anxiety or social phobia is more common than common female.The importance of mental health is emphasized in the International Evidence-Based Guideline for the Assessment and Management of Polycystic Ovary Syndrome in 2018.Objective: to investigate the diversity of psychiatric disorders among different PCOS phenotypes and the relationship between the severity of psychiatric disorder and glucose and lipid metabolism.Methods: 167 patients with PCOS who met the Rotterdam Standard and had not been treated were enrolled.The levels of basal reproductive endocrine hormone,blood glucose and insulin,blood lipid and vitamin D were measured,according to the phenotypes of PCOS,the patients were divided into four groups:(1)ovulatory disorder + hyperandrogenism(OA + HA)group,(2)OA + polycystic ovaries on ultrasound examination(PCO)group,(3)OA + HA + PCO group,(4)HA + PCO group.Results:(1)53.9% of the patients in this case group had depression disorder(22.2%of the patients were moderately or above),and 52.7% had generalized anxiety disorder(17.4% of the patients were moderately or above)(2)The risk of depression disorder in OA + HA + PCO group was higher than that in other groups.(3)The level of insulin in moderate or above depression disorder group and GAD group was higher than that in the other two groups.(4)the level of HDL in patients with moderate or above GAD level was lower than that in normal group(p < 0.05);(5)the level of Vit D in patients with moderate or above depression disorder was lower than that in normal group(p < 0.05).Conclusion: psychiatric disorder or impaired glucose and lipid metabolism are common in patients with PCOS.According the PCOS phenotypes,OA + HA + PCO phenotype has higher long-term risk of psychiatric disorders,metabolic abnormalities,in clinical work should be paid attention to.Vit D levels may be used to assess the level of depression in patients.
