Correlation Analysis Of Fasting Plasma Glucose Coeffocoent Of Variation And Diabetic Retinopathy In Patients With T2DM In Qinghai Area

Purpose:To explore the correlation between the variation of fasting blood glucose and diabetic retinopathy in T2DM patients in Qinghai area,and to clarify its diagnostic and prognostic value for diabetic retinopathy.Methods:Randomly selected data integrity in September 2018-September2020 clinic allocated about university hospital endocrinology and is in line with the inclusion criteria and exclusion criteria,580 patients with type 2 diabetes,according to whether the merger diabetic retinopathy and the degree of retinopathy is divided into:no diabetic retinopathy group(NDR group)301 cases,non-proliferative diabetic retinopathy group(NPDR group)150 cases,proliferative diabetic retinopathy group(PDR group)129 cases.The general data of the patients,Hb A1c,TC,TG,WBC,Hb,PT,MALB,FPG-CV and the results of fundus imaging were collected.SPSS23.0 statistical software was used for data analysis.Results:(1)There were statistically significant differences in age,duration of diabetes,BMI,TG,Hb A1c,MALB,24-hour urinary protein,FPG-CV,WBC,RBC,HB,PLT,PT and APTT among the three groups(P<0.05).There were no significant differences in gender,history of hypertension,smoking history,TC,altitude of residence and atmospheric oxygen content among the three groups(P>0.05).(2)Spearman correlation analysis of the three groups showed that the duration of diabetes(r_s=0.673),FPG-CV(r_s=0.650),Hb A1c(r_s=0.613),age(r_s=0.428),MALB(r_s=0.252)were positively correlated with DR.Hb(r_s=-0.277)and PT(r_s=-0.400)were negatively correlated with DR,and the difference was statistically significant(P<0.05).(3)Patients with T2DM were grouped according to whether they were combined with DR.Bariate Logistic regression analysis was performed on the influencing factors of the occurrence of DR for diabetes course,FPG-CV,Hb A1c,age,PT,Hb,MALB,etc.The results showed that FPG-CV(OR=4.537,95%CI:2.060,9.994),Hb A1c(OR=3.759,95%CI:2.498,5.656)and duration of diabetes(OR=1.799,95%CI:1.503,2.153)were risk factors for retinopathy in patients with type 2 diabetes mellitus.PT(OR=0.399,95%CI:0.326,0.742)were the protective factors for retinopathy in type 2 diabetes(P<0.05).(4)Patients with T2DM were grouped into groups according to the degree of DR lesions,and a binary Logistic regression analysis was conducted on the influencing factors of diabetes course,FPG-CV,Hb A1c,age,PT,Hb,MALB,etc.The results showed that Hb A1c(OR=1.545,95%CI:1.244,1.919)and age(OR=1.074,95%CI:1.039,1.109)were risk factors for the development of type 2diabetic retinopathy,and PT(OR=0.285,95%CI:0.029,0.349)was a protective factor for the development of type 2 diabetic retinopathy(P<0.05).Conclusion:(1)FPG-CV,duration of diabetes and Hb A1c are independent risk factors for the occurrence of DR in T2DM patients in Qinghai area.Age and Hb A1c were independent risk factors for the development of DR,while PT was a protective factor for the occurrence and development of DR.(2)Age and course of disease of diabetic patients are uncontrollable factors,but clinical monitoring of Hb A1c level,fluctuation of fasting blood glucose and PT level can be used to prevent and cure DR.(3)The duration of diabetes,FPG-CV,Hb A1c,age and MALB were positively correlated with DR,while Hb and PT were negatively correlated with DR.(4)The severity of DR increased with age,course of diabetes,increase of Hb A1c and decrease of Hb and PT.The occurrence of DR was related to TG,MALB,24 h urinary protein and FPG-CV level.
