Objective: to determine the relevant risk factors of diabetic retinopathy through the analysis on the relationship between diabetic retinopathy and glucose fluctuation,in order to provide more theoretical basis for the clinical treatment on diabetes,to confirm whether the stable glucose control is equally important as the mere control of mean value of blood glucose or not,and whether it is conducive to relieve or delay the occurrence of diabetic chronic complication or not.Methods: 128 cases of 2 type diabetic patient admitted in endocrinology department of the Second Hospital of Hefei from 2014 to 2015 were selected as object of study,they were divided into NDR group and DR group according to whether a diabetic retinopathy is present,the patients' age,gender,course of diabetes,blood pressure,body mass index(BMI),glycosylated hemoglobin(Hb A1C),total cholesterol(TC),triglyceride(TG),high-density lipoprotein(HDL)and low-density lipoprotein(LDL)were recorded,and a successive 72-hour blood glucose monitoring had been performed on the patients,the patients' mean fasting blood glucose,all-day mean blood glucose and glucose fluctuation coefficient had been recorded,the correlation between preceding indexes and risk of diabetic retinopathy was analyzed.Results:.the difference in age,illness course and fluctuation coefficient between the two groups was equally statistically significant(P<0.01),so was the difference in systolic pressure(P<0.05),but the difference in glycosylated hemoglobin(Hb A1C),total cholesterol(TC),triglyceride(TG),high-density lipoprotein(HDL)and low-density lipoprotein(LDL),diastolic pressure and mean fasting blood glucose between the two groups was equally not statistically significant(P>0.01).The result of Logistic analysis showed that,the diabetic patients' illness course,systolic pressure and glucose fluctuation coefficient were associated with the occurrence of diabetic retinopathy,they may be the independent risk factors of diabetic retinopathy.Conclusion: the diabetic patients' illness course,systolic pressure and glucose fluctuation coefficient were the independent risk factors of diabetic retinopathy.In the process of blood glucose control,attention should be paid to the stable reduction of blood glucose and control of blood glucose fluctuation coefficient,particularly,among the patients who had a long diabetic course,a special attention should be paid to the amplitude of blood glucose fluctuation of patients,so as to relieve and delay the occurrence of diabetic retinopathy;among the patients who had a diabetes combined with hypertension,the effect of blood pressure control on reducing the risk of diabetic retinopathy cannot be ignored either.
