Objective: Establish rabbit model,the tendon surgery to repair the postoperative application of BMSCs,PRP,bFGF,observe the quality of the Achilles tendon healing,the influence of the strength of healing and preventing adhesion,seek a more effective exogenous growth factors promote tendon healing,reduce scar hyperplasia,enhance the Achilles tendon biology performance,provide new ideas and experimental basis for clinical application.Methods: Sixty-five healthy rabbits(male,body weight 2.5-2.8kg)were randomly divided into group A: BMSCs group,group B: PRP group,group C:bFGF group,and group D: control group(given normal saline),with 15 rabbits in each group.Five rabbits were used to prepare BMSCs.The left hind leg Achilles tendon of the experimental animal was cut off,and the broken end was sutured by Kessler suture.Exogenous growth factors were given to the experimental group after suture and1,2 and 3 weeks after operation.Results: No postoperative wound infection occurred in each group.Gross observation: the range of motion of left lower limb ankle joint in the experimental group was significantly better than that in the control group,and the scar around the Achilles tendon in the experimental group was significantly less than that in the control group.BMSCs group was better than PRP group,and PRP group was better than BFGF group.The biomechanical test results showed that the average maximum tensile resistance,average maximum tensile length and average stress value of Achilles tendon in BMSCs,PRP and BFGF groups were significantly different from those in the control group at 4 weeks after surgery,and the differences were statistically significant(P<0.01),and BMSCs group was superior to PRP group,and PRP group was superior to BFGF group.Histological observation results: 4 weeks after surgery,microscopic observation with HE staining showed that a large number of fibroblasts and a small number of inflammatory cells could be seen in the Achilles tendon fracture in BMSCs group,and the collagen fibers were arranged in a relatively orderly undulated manner.In PRP group,the tissue structure was slightly abnormal and the arrangement of collagen fibers was disordered.In bFGF group,the tissue structure was abnormal,the fiber arrangement was relatively disordered,and the number of inflammatory cells was more,and a few capillaries were visible.The number of inflammatory cells in the control group was significantly higher than that in the other three groups,the growth of fibroblasts and the content of myogenic protein in the control group were significantly lower than that in the other three groups,and the arrangement was disorderly,and the granulation tissue growth was more.Immunology organized staining results show that the postoperative 4 weeks Ⅰ type collagen fiber is positive expression,and BMSCS group Ⅰ type,the most abundant collagen fiber number expression PRP slightly more than bFGF group and control group in the least.Conclusion: the number of collagen fibers and the strength of biomechanics in the experimental group are significantly higher than those in the control group,which can promote the healing of Achilles tendon,increase its healing strength,and effectively prevent adhesion.BMSCs had the best effect,BMSCs had better effect than PRP,and PRP had better effect than BFGF.Therefore,in the application of exogenous cofactors,BMSCs can better promote Achilles tendon healing and prevent scar hyperplasia.
