Objective: To study and analyze the clinical value of magnetic resonance image composition(Magic)on diagnosis of multiple sclerosis(MS),by measuring the lesion area of multiple sclerosis,the normal appearance white matter area and cerebral small vessel disease lesion area.Methods: 36 patients with multiple sclerosis(male: 9,female: 27),20 patients with cerebral small vessel disease(male: 5,female: 15)and 30 healthy volunteers matched with MS group in age and gender were collected.Of the 36 MS patients,15 patients underwent plain scan and enhanced scan,while the remaining 21 MS patients,20 CSVD patients and 30 healthy controls underwent plain scan only.All cases were scanned by magic technology.The T1 relaxation time,T2 relaxation time and proton density in the central and peripheral areas of multiple sclerosis lesions,the lesion areas of cerebral small vessel disease,the normal appearance white matter areas of MS patients and the corresponding areas of healthy control group were measured in the automatically generated T1 mapping,T2 mapping and PD mapping images.The differences between the central and peripheral areas of multiple sclerosis lesions,the normal appearance white matter areas of multiple sclerosis and healthy control group,multiple sclerosis and cerebral small vessel disease were compared.At the same time,the ROC curve of each parameter was analyzed to calculate the sensitivity and specificity of diagnosis of multiple sclerosis.Results: the values of q T1,q T2 and PD in the central area of MS were2243.24±486.211 ms,185.04±44.669 ms and 100.141±4.0756 pu,respectively;the values of q T1,q T2 and PD in the peripheral area of MS were 1304.11±165.070 ms,113.15±13.405 ms and 87.715±7.730 pu,respectively.The values of q T1,q T2,PD in the centrum were higher than those in the periphery.The results of before and after enhancement of the same lesion in active MS,the values of q T1,q T2 and PD were 2799.13±213.332 ms,249.33±30.617 ms and 123.300±6.411 pu respectively on plain scan,and 474.87±29.272 ms,134.07±18.847 ms and 104.027±0.837 pu respectively on enhanced scan.The values of q T1,q T2 and PD on plain scan were higher than those on enhanced scan.The q T1,q T2 and PD values of the centrum semiovale and lateral ventricle NAWM in MS group were 866.75±47.473 ms,90.88±7.099 ms and 68.58±3.407 pu,respectively.The q T1,q T2 and PD values of the two regions in healthy control group were 755.29±31.037 ms,76.00±8.648 ms and 65.675±1.953 pu,respectively.The q T1,q T2 and PD values of NAWM in MS were significantly higher than those in healthy control group.The values of q T1,q T2 and PD were 1509.11± 456.964 ms,128.03±37.049 ms and 91.973±7.781 pu in MS group,and 937.78±78ms,100.43±6.543 ms and 72.019±4.793 pu in CSVD group.The values of q T1,q T2 and PD in MS group were higher than those in CSVD group.The ROC curves of q T1,q T2 and PD were made in 36 patients with Ms.the results showed that q T1(AUC = 0.990)> PD(AUC = 0.982)> q T2(AUC =0.818).AUC > 0.7 in all three groups.The sensitivity and specificity of q T1 were96.7% and 91.3% for MS.The sensitivity and specificity of q T2 were 68.6% and60.5%.And the sensitivity and specificity of PD were 92.5% and 89.8%.Conclusion: the parameters of magic scan can detect the potential damage that can not be detected by conventional MRI,and improve the early diagnosis of MS patients.The diagnostic efficiency of the three groups are very high,and it has important clinical value in the differentiation of multiple sclerosis and cerebral small vessel disease.
