Objective:Rheumatoid arthritis is a disease that accounts for a large proportion of patients in rheumatology wards.The number of patients with joint deformity caused by RA disease activity is increasing every year.Therefore,it is necessary to actively search for indicators that can diagnose or predict disease activity early.In the past,indicators related to disease activity,such as C-reactive protein and anti-cyclic citrullinated peptide antibodies,are susceptible to interference from other factors,so the results are not always accurate.This study intends to analyze the correlation and clinical value of serum matrix metalloproteinase 3 and rheumatoid arthritis,as well as the diagnostic efficacy of early RA,and to explore whether the level of MMP-3 can be used as a potential marker of the severity of RA.Methods:We selected 92 RA cases who came to our hospital’s rheumatology and immunology ward from October 2019 to October 2020,including 13 males and 79females,with a median age of 56.90±13.30 years.During the same period,42 people came to the hospital for physical examination,including 10 males and 32 females,with a median age of 55.10±0.89 years.The fasting peripheral blood of RA patients and healthy subjects was collected with a serum tube with a separating gel,and the specimens were stored in a refrigerator at-70℃and kept at low temperature.Using the ELISA method,using the C16000 automatic biochemical immunoassay analyzer(U.S.Abbott,latex enhanced immunoturbidimetric method)at a relative centrifugal force of 3,000×g for 10 min,the serum was separated,and the serum MMP-3 was uniformly detected.Collect clinical data of RA patients,including ESR,CRP,RF,CCP,Fe2+levels,and calculate DAS28 score.Using t test,Pearson correlation analysis,analysis of the differences between the two groups,correlation analysis and ROC curve drawing to evaluate the clinical diagnostic significance of MMP-3 to RA,and to explore the correlation between MMP-3 and RA disease activity.Results:The serum MMP-3 level in the RA group was(196.35±304.59)ng/ml,which was significantly higher than that of the control group(40.17±19.91)ng/ml,and the difference was statistically significant(t=-3.313,P<0.001).The area under the ROC curve AUC was 0.741(95%CI:0.658~0.813).When 60.0ng/ml was the best cut-off point,the sensitivity of MMP-3 in diagnosing RA was 60.87%and the specificity was90.48%.Serum MMP-3 in the RA group was significantly positively correlated with the number of joint swellings in CRP,ESR,and DAS28(r values were 0.29,0.25,0.283,and P values were all<0.05),but not correlated with CCP,RF,Fe2+(r value Respectively-0.177,-0.094,-0.322,P values are all>0.05).In the RA group,the MMP-3 results of the low activity group and the moderate and high activity groups distinguished by DAS28scores were compared,and the difference was statistically significant,and the P values were all<0.05.Conclusion:The level of MMP-3 in the serum of patients with active RA was significantly increased,and the level of MMP-3 was higher than that of the control group.The diagnosis of RA showed good clinical efficacy.The level of MMP-3 is correlated with RA disease activity,especially with the number of joint swelling and pain,which is of great significance in evaluating disease activity and prognosis in RA patients.
