Study On Mechanism Of Calcium Dobesilate Combined With Irbesartan To Reverse The Early Diabetic Nephropathy

Objective:Discuss the clinical effect of calcium dobesilate combined with irbesartan on reversing the early diabetic nephropathy,providing a new strategy for the early prevention and treatment of diabetic nephropathy.Methods:A total of 150 patients with type 2 diabetes complicated with early diabetic nephropathy were selected as subjects.They were randomly divided into groups A,B and C,each with 50 cases.During the period,the patients kept the original suitable diet,exercise,hypoglycemic drug treatment relatively stable,with blood glucose of the patients effectively controlled.On this basis,patients in group A were treated with irbesartan at a dose of 0.15g,once/day;patients in group B treated with calcium dobesilate at a dose of 0.5g,three times/day;group C treated with calcium dobesilate combined with irbesartan at the same dose as group A and B.12 weeks was a course of treatment,a total of 24 weeks.24 h urine protein in patients with quantitative(24-hour urinary protein quantity),uminuriamicroalb(UMA),urine albumin/creatinine(ACR),urine β2-microglobulin(β2-MG),s eru m cystatin C(Cys C),serum inflammatory mediators(hs-CRP),blood urea nitrogen(BUN),serum creatinine(Scr),fasting blood glucose(FBG),postprandial 2 h blood glucose(2hPG),glycosylated hemoglobin(HbAlc)were detected in before treatment and 12 weeks and 24 weeks.Besides observing the presence of adverse reactions.The clinical effect of calcium dobesilate combined with irbesartan on early diabetic nephropathy was determined by comparing the changes of various indexes before and after treatment.Results:Patients in the three groups treated for 12 weeks and 24 weeks(1)24-hour urine protein quantification,urine microalbumin,urine microalbumin/urinary creatinine,urine β2-microglobulin,and cystatin C were decreased before treatment,and the difference was statistically significant(P<0.05);(2)There was no statistical difference between group A and B(P>0.05).Besides,the group C had a more significant decline than the A and B,and lower than them(P<0.05);(3)Other indicators were not changed significantly compared with before treatment.Conclusion:Calcium dobesilate combined with irbesartan in the treatment of early diabetic nephropathy has a more obvious clinical effect,even reversing the progression of nephropathy;besides,the longer the treatment time,the greater the effect.
