Objective To observe the therapeutic effect of calcium dobesilate (calcium dihydroxy 2, 5-benzenesulfonate) on diabetes mellitus with early nephropathy in order to find a new therapeutic approach to diabetic nephropathy.Methods 67 cases of type 2 diabetic nephropathy (albuminuria: 30 to 300 mg/24h,male/female:37/30, mean ages: 55 years old, mean course of disease: 11.8 years) without hypertension were selected.Some indexes were analyzed before and after administration of regular therapy , perindopril(4mg qd) or calcium dobedilate (500mg tid) when the metabolism is stable.We measured the levels of the indexes including blood pressure , fasting serum glucose, HbAlc, fasting serum C peptide, TG, CH, LDL-C, HDL-C, ALT, Y -GT, BUN, Cr, Hct, blood viscosity, fibrinogen (Fbg) , plasminogen activator inhibitor-1 (PAI-1) , endothelin (ET) , Netric oxide(NO), urinary albumin excretion rate (UAER) ,and tested UAER once a month during the observation.Results After three months treatment , the levels of UAER were decreased significantly in both dobesilate group and perindopril group(P<0.01). During the three months UAER was lowering step by step ,and there is no difference between them. The levels of UAER didn't change in regular group and there is significant difference between it and the former two groups.In dobesilate group,the levels of Whole blood viscosity of medium shear rate 1, whole blood viscosity of medium sheer rate 2, whole blood viscosity of low shear rate were declined and serum NO increased significantly ;that of plasma viscosity , whole blood reduced viscosity, PAI-1 , Fbg , ET were all decreased to some degree. The levels of ET in perindopril group delinedY -GT, BUN, Cr, fasting serum C peptide, Hct and blood pressure had no change in three groups before and after observation .Conclusion Calcium dobesilate has the therapeutic effect on type 2 diabetic nephxopathy patients with microalbuminuria because of its effect of decreasing UAER and it can postpone or reverse the development of DN.The benefit was positively correlated with the time of taking calcium dobesilate . The therapeutic effect of calcium dobesilate may be due to the decreases of blood viscosity ,PAI-1 , Fbg , ET , and the increase of NO which were induced by calcium dobesilate.
