Objective: To evaluate the differences and correlations of subjective and objective psychological stress of gastroesophageal reflux disease(GERD)patients and normal people,non-erosive reflux disease(NERD)and reflux esophagitis(RE)patients,as well as different grades of RE patients, and to analyze the relationship of these two psychological stress and symptom, and also to combine these two kinds of psychological stress with some living and eating habits to find risk factors of GERD.To investigate the effects of subjective and objective psychological stress on patients with different subtypes and severity. And then combing with the above results, taking subjective psychological stress as the benchmark,to comprehensively investigate the application value of objective psychological stress which used in GERD patients. Methods: A total of 137 patients diagnosed as GERD in our outpatient clinic from June 2019 to September 2020 were selected as the GERD group, and 47 healthy people matched with the gender and age of GERD group were picked as the control group.According to subtypes of GERD,the GERD group was divided into NERD group(n=50)and RE group(n=87).And in the light of the Los Angeles classification(LA),RE patients were classified into RE A(n=57)、RE B(n=22)and RE C group(n=8).Used psychological stress assessment software(Anura V2.0.6)which based on transdermal optical imaging technology(TOI)to measure objective psychological stress score(Anura stress index,ASI value)and defined ASI value≥3.3 as greater objective psychological pressure;and used Chinese perceived stress scale(CPSS)to evaluate subjective psychological stress score(CPSS value)and defined CPSS value≥26 as greater subjective psychological pressure.Assessed symptom degree of GERD patients(Gerd Q value)by using the Gastroesophageal reflux disease questionnaire(Gerd Q),and also collected the general clinical data of all subjects. Finally, our research used SPSS 23.0 software to perform statistical analysis on the above data. Results: Through analysis, there were inexistence remarkable discrepancies in age and gender proportion between GERD and control group,NERD and RE group,as well as each subgroup of RE,the above groups have comparable(P>0.05).1.Differences in subjective and objective psychological stress of each group:(1)GERD and control group: the ASI,CPSS value and the proportion of people with ASI value≥ 3.3 of GERD patients were significantly greater than control group(P <0.05),and the proportion of people with CPSS value ≥26 in these two groups had no statistical difference(P>0.05);(2)NERD and RE group: the ASI,CPSS value and the proportion of people with ASI value ≥ 3.3 of NERD group were significantly exceed than RE group(P<0.05),the proportion of people with CPSS value ≥26 in these two groups had nondistinction difference(P>0.05);(3)Subgroups of RE: there was only one significant difference between the subgroups, which was the ratio of people with ASI value ≥ 3.3(RE B group > RE C group > RE A group)(P<0.05).2.Pertinence of subjective and objective psychological stress of each group: in the GERD group,NERD group,RE group and control group, the ASI value and the CPSS value were all remarkably positive correlation(P<0.05).3.Risk factors of GERD: this research found that higher ASI value, drinking history, preference sweet or unripe food, preference cold food as well as food intake 3 hours before bedtime(≥3 days/week)were the independent pathogenic factors of GERD.4.Symptom score of GERD patients and its affiliation with subjective and objective psychological stress: NERD group had higher Gerd Q score than RE group(P<0.05),there was no significant difference among the subgroups of RE patients(P>0.05);In GERD group, the Gerd Q value was positively correlated with ASI value(P<0.05),but had no relationship with CPSS value.IV Conclusion: 1.GERD patients have greater subjective and objective psychological pressure than healthy people;according to the subtypes of GERD,NERD patients have sustained more subjective and objective pressure than RE patients;according to the LA classification, these two pressures of RE patients have no noteworthy differences.2.The objective psychological stress is an independent pathogenic factor of GERD,but the subjective psychological stress is not the independent risk, and the former stress is more closely associated with GERD.3.The objective psychological pressure measured by transdermal optical imaging technology software is positively correlated with subjective psychological pressure measured by traditional psychological scale, the former can replace the latter to some extent. In the meantime, the objective psychological stress also has a stronger correlation with GERD subtypes and symptoms,so our research infers that transdermal optical imaging technology can may more objectively and accurately evaluates mental stress of GERD patients,it can be a scientifically method for measuring psychological stress in GERD patients.
