Objective:To explore the correlation analysis between the changes of renal function and Serum TNF-α、IL-1β、IL-6 in Patients with Type 2 Diabetic kidney disease,and provide clinical evidence for the treatment of diabetic nephropathy and the study of its pathogenesis.Methods:1.Retrospectively analyze the medical history data of T2 DM patients hospitalized in the Department of Nephrology、Endocrinology and Metabolism of the China-Japan union hospital from January 2019 to January 2022.2.Grouping:The patient case data were collected and divided into six groups: Healthy physical examination patients were set as blank control group A(n=13),patients with pure type2 diabetes were set as B group(n=14),and according to Mogensen diabetic nephropathy staging criteria,I-II stage was set as C group(n=17),patients with stage III were set as group D(n=25),patients with stage IV were set as group E(n=18),and patients with stage V were set as group F(n=20).3.Record and collect general clinical data and laboratory examination indicators of the above patients who meet the standards,including gender、age、diabetes course、body mass index(BMI)、fasting plasma glucose(FPG)、glycated hemoglobin(Hemoglobin a1 c,Hb A1c)、triglycerides(TG)、total cholesterol(TC)、low density lipoprotein cholesterol(LDL-C)、Urinary albumin /creatinine ratio(UACR)、Serum creatinine(s Cr)、24-hour urine protein quantification、Serum α1-microglobulin、urine α1-microglobulin、urinary transferrin and other biochemical indicators,using enzyme-linked immunosorbent assay(ELISA)to detect serum TNF-α、IL-1β、IL-6 levels.4.The data uses SPSS 25.0 software,and chi-square test was used for comparison between groups;variance analysis was used between multiple groups;Spearman method was used to analyze the correlation between serum TNF-α 、 IL-1β 、 IL-6 and other indicators;P <0.05 means the difference is statistically significant.Results:1.Comparing the general clinical data of the six groups of patients,the results showed that males accounted for 61.7% of the total,and the prevalence was greater than that of females.Meantime,there was no significant difference between the age groups(p>0.05),however there was a statistical difference in the course of DM and BMI between the groups(p<0.05),that is,the longer the course of DM and the more obese patients were more likely to progress to DKD;2.Comparing the biochemical indicators of the six groups of patients,DKD patients showed clinical features such as increased creatinine 、 blood and urineα1-microglobulin、and urinary transferrin,which were positively correlated with DKD stages;3.Using spearman to analyze the correlation between UACR and various indicators,it can be concluded that:UACR level is related to the course of DM、glycosylated hemoglobin 、 total cholesterol 、 low density lipoprotein 、 serum creatinine 、 24-hour urine protein quantification 、 urine microalbumin 、 Serumα1-microglobulin、urine α1-microglobulin、random urinary microalbumin and urinary transferrin were significantly correlated,but not correlated with BMI、 fasting blood glucose and triglyceride;4.Serum TNF-α、IL-1β and IL-6 were all expressed in DKD group and T2 DM group,and the expression was significantly higher than that in the control group and T2 DM group,and with the progression of DKD,the expression level gradually increased.The analysis was compared between groups,and the difference was statistically significant(p<0.05);5.The level of serum TNF-α、 IL-1β and IL-6 in group F is the highest in stage V patients,followed by stage V、stage III、stage I-II.There is no expression in the blank control group.Statistical significance(P<0.05);Kendall’s tau-b correlation analysis showed that serum TNF-α、 IL-1β、IL-6 levels were positively correlated with the clinical stage of DKD patients(r>0,P<0.05).Conclusion:1.UACR level and duration of DM,glycosylated hemoglobin,total cholesterol,low density lipoprotein,serum creatinine,24-hour urine protein quantification,bloodα1-microglobulin,urine α1-microglobulin,random urine microalbumin,urinary transferrin There was a significant correlation,and 24-hour urine protein quantification and blood α1-microglobulin were independent influencing factors of ACR.2.The expression of TNF-α,IL-1β and IL-6 can be detected in the serum of patients with type 2 diabetic nephropathy,and the expression level is positively correlated with the DKD stage.The higher the clinical stage of the patient.
