Abstract/Summary:
PurposeTo investigate the relationship between hepatitis B virus and the occurrence and development of rectal cancer by analyzing the status of hepatitis B virus infection in rectal cancer.MethodThis is a multicenter, retrospective study. We collected data about rectal cancer, traumatic fractures, hepatocellular carcinoma from three hospitals in Guangdong Province from 2007-2016.R software 3.3.1 was used to calculate the standardized incidence(SIR)(SIR was defined as the ratio of observation to expected value)and the 95%confidence interval of SIR.If "1" was within the confidence interval, the result was no statistical Differences, and "1 " is not within the confidence interval, the results were statistically significant.Wilcoxon rank sum test with continuity correction for mean age between HBV-and HBV+,P<0.05 was considered statistically significant. All statistical analyses were performed using R software 3.3.1Result1,Compared with national rate,HBV play a role in promoting the rectal cancer.5309 patients of rectal malignancies were recruited with SIR 1.3(95%CI:1.2-1.4).Results from the three hospitals show the same trend. The SIR for liver cancer with HBV infection was 10.0(95%CI:9.8-10.1)and was 1.1(1.0-1.2)for traumatic fracture.2,The mean age of rectal cancer patients with HBV infected is younger than that of non-infected HBV.The results of both men and women showed consistency, mean age with a difference of approximately 3.1-4 years. The average age in patients with HBV+and rectal cancer in the three hospitals was 56.0± 12.6,and the average age of non-HBV was 59.8± 13.2.(P<0.001);In the male rectal cancer group, the infection group was 56.3± 12.1 and non-infected were At 60.3± 13.0(P<0.001);In the female colon cancer group,55.5± 13.5 were infected and 58.6 ± 13.4 were noninfected(P=0.003).3,HBV are related with rectal cancer in different area.In Guangdong and nonGuangdong population, the normalized SIR(95%CI)for HBV associated with rectal cancer was 1.3(1.2-1.4)and 1.5(1.2-1.8),respectively.4,The results showed consistency in well/moderate/poor histological differentiation in patients with rectal cancer.Overall, the SIR(95%CI)associated with HBV in well/moderately/Poorly differentiated adenocarcinoas was 1.4(1.2-1.5)and 1.3(1.2-1.5)and 1.4(1.1-1.9),respectively.5,HBV infection had a certain relationship with the Ⅲ clinical stage of rectal cancer.1773 patients of rectal malignancies were recruited and 687 were patients with clinical Grade Ⅲ rectal cancer. For the rectal cancer patients with stage records, the SIR of Ⅲ stage were 1.5(1.2-1.9)with statistical difference.SIR(95%CI)of rectal cancer stage 0 were 0.8(0.1-3.0)and stage Ⅰ were 1.1(0.7-1.5)and stage Ⅱ were 1.3(1.0-1.7)and stage Ⅳ were 1.2(0.8-1.7),both without statistical difference.Conclusion1,HBV may promote the occurrence and development of rectal cancer, but the relationship is weaker than between HBV and liver cancer.2,The mean age of HBV infected rectal cancer group was younger than that of HBV-group.It is possible that HBV may lead to become younger for rectal cancer.3,In different histological differentiation(well/moderate/poor),different geographical regions(Guangdong Province and non-Guangdong Province),HBV may have a certain relationship with the occurrence of rectal cancer.4,HBV may be associated with III rectal cancer and lead to malignancy of rectal cancer.
