Objective:To analyze the basic situation of hospitalization expenses for patients with viral hepatitis from 2015 to 2017,and then explore the key factors affecting the change of hospitalization expenses,so as to provide decision-making basis for reducing the personal burden of patients,and to provide methodological reference for the study of related disease costs.Methods:The data source of this study consists of two parts:one is from the health expenditure research literature,and the other is the discharge time of 58 Chinese medicine hospitals in a province from January 1,2015to December 31,2017,and the main diagnosis is the reality of patients with viral hepatitis’s real world medical record home page,data analysis using SPSS22.0 and Amos21.0,using the optimal scale regression and structural equation modeling to fit the influencing factor model.Results:According to literature research,it can be found that the research hotspots of pre-medical expenses focus on the discussion of medical insurance.With the reform of medical and health care,researchers began to find ways to control the increase of medical expenses in a quantitative way.The influencing factors have become the current hot research topic.The 50%hospitalization cost of patients with viral hepatitis in this study was more than 6,000 yuan,and the proportion of Chinese and Western medicine expenses was the highest in all hospitalization expenses,accounting for 34.32%of the total cost,followed by diagnostic fees and other expenses.These three items accounted for more than 70%.Univariate analysis showed that 12 factors including hospitalization day,hospitalization,hospital level,medical payment method,gender,age,occupation,admission route,treatment category,whether to implement clinical pathway,and whether to use Chinese medicine preparations in medical institutions were significant for hospitalization expenses.Impact(P<0.05).The regression equation obtained by the optimal scale regression is:Y1=0.342Y2+0.203X9-0.715X1+0.096X8-0.073X2-0.06X6+0.062X5-0.047X7+0.029X3,Among them,the in hospital stays(total effect size 0.342),the percentage of the drug expenditure(total effect size 0.246),and the hospital grade(total effect size 0.175)are the main factors affecting the hospitalization expenses,except that the gender has no direct impact on the total hospitalization expenses,and other variables are related to the total hospitalization.The results of the structural equation model show that the total effect of each indicator on the total cost of hospitalization is from large to small:the in hospital stays(0.403),the percentage of the drug expenditure(0.107),hospital grade(0.089),Whether to use medical institutions Chinese medicine preparations(0.058),hospitalized times(0.024),age(0.027),admission path in hospital(0.019),occupation(0.016),gender(0.016),the type of medical insurance(0.003).The in hospital stays can have a direct impact on the total hospitalization expenses,and can also play the role of intermediate variables,other variables indirectly affect the total hospitalization expenses through the hospitalization day.Conclusions:This study analyzed the hospitalization expenses of 8421patients with viral hepatitis and found that the variables affecting the total cost of hospitalization include hospital grade,the type of medical insurance,admission path in hospital,age,occupation,gender and whether to use traditional Chinese medicine preparations,the percentage of the drug expenditure and the in hospital stays.In this paper,some potential variables,such as clinically relevant indicators,are not included.Later,further research should be conducted to improve the model.Multivariate analysis modeling fits the results of influencing factors of hospitalization expenses for patients with viral hepatitis.It is ideal that can be further extended to other disease cost studies.As a hot research topic,medical expenses are aimed at the problem of medical cost control.Chinese researchers have discussed the characteristics of hospital management,social economy,etc.,but in addition to hospitals and patients,the role of government-oriented medical cost control is also worthy of causing Concerned.
