Clinical Effect Of Phacoemulsification And Goniosynechialysis In Primary Angle-Closure Glaucoma With Co-existing Cataract

Abstract/Summary:
Purpose: To observe the treatment effect of phacoemulsification with goniosychialysis in primary angle-closure glaucoma with co-existing cataract,and to evaluate the clinical effect and safety of this surgical methods.Methods: 39 patients(46 eyes)of PACG with co-existing cataract;including 9men,30 women.The patient with the history of intraocular pressure(IOP)≥40mm Hg are distributed to group A(15 eyes).<40mm Hg are distributed to group B(31 eyes).Both group were treat by phacoemulsification intraocular lens implantation and goniosynechialysis(Phaco+IOL+GSL)after reducing the IOP.Analyze the visual acuity,IOP,anterior chamber depth(ACD),and anterior chamber angle.And discuss the treatment effect and influence with ocular hypertension before the Phaco+IOL+GSL surgery.Results: The postoperative best corrected visual acuity in group A and group B was significantly improved compared with preoperative.The postoperative visual acuity difference was statistically significant(P<0.05)with preoperative in group A and group B.The difference with the visual acuity in group A and group B postoperative were statistically significant(P<0.05).The visual acuity preoperative in Group B are more improved then group A.After the reducing of IOP,the average IOP preoperative in group A and group B shows no siginificant difference(P>0.05).The postoperative IOP of group A and group B were statistically significant(P<0.05)with preoperative.The IOP postoperative with group A and group B were not statistically significant(P>0.05).The ACD preoperative with group A and group B were statistically significant(P<0.05).The ACD postoperative with group A and group B were not statistically significant(P>0.05).The postoperative ACD in group A and group B were deeper then preoperative(P<0.05).The postoperative anterior chamber angle in group A and group B were opened significantly compared withpreoperative(Z=8.852,P<0.05).There are 14 cases out of 15 has the anterior chamber angle all open in group A.1 case of group A has <180°anterior chamber angle closure.All 31 cases in group B has the anterior chamber angle all open.Conclusion: In primary angle-closure glaucoma,phacoemulsification and intraocular lens implantion and goniosynechialysis got the good treatment effect in improve patient’s visual acuity,reduce intraocular pressure,deeper the anterior chamber depth,re-open the anterior chamber angle.Also got the good treatment effect in patient with high intraocular pressure.For the treatment of primary angle-closure glaucoma with co-existing cataract,this surgery is effective and safe.
