Objective The objective of this study was to intervene essential hypertension(EH)patients by Baduanjin exercise for 24 weeks.By testing in patients with EH platelet Arg-Ⅱ protein expression and NOS activity,NOS expression,NO levels and blood pressure to compare the intervention group with the control group of patients with the change of related indicators,clarify its change rule and trend from the perspective of L-Arg /NOS/NO pathway which could provide basic research evidence for the clinical application of Baduanjin.Methods The study samples were 62 EH patients who visited the department of cardiology of People’s Hospital Affiliated to Fujian University of Traditional Chinese Medicine from September 2017 to August 2018.The patients were randomly divided into two groups,namely the intervention group(Baduanjin exercise group)and the control group(31 cases each).The control group received routine treatment and health education.On this basis,the intervention group received Baduanjin exercise intervention.Patients should exercise 5 days a week,once a day,for two times,each time exercise for 13 minutes and rest for 5 min between times,about 30 min a time,for a total of 24 weeks.Before intervention,after 24 weeks of intervention,the blood pressure,Arg-Ⅱ protein expression and activity of NOS,NOS and NO level of protein expression changes in the two groups would be observed and recorded for analysis;blood pressure changes of the patients every 4 weeks after the intervention were evaluated.ResultsThis study included 62 patients,finally completed the subjects intervention group 26 cases and control group 27 cases.1.Baseline ComparisonGeneral information of the patients in the two groups before intervention: gender,age,marriage,height,weight,and education level were not statistically significant(all P>0.05).There was no significant difference in family history of EH,course of EH,grade of EH,history of hyperlipidemia,complications or medication between the two groups(all P> 0.05).There was no significant difference in smoking,drinking,tea drinking and diet between groups(all P> 0.05);During the intervention period,there was no significant difference in the use of antihypertensive drugs,lipid-lowering drugs and traditional Chinese medicine decoction/granules between the two groups(all P>0.05).Arg-Ⅱ protein expression and activity of NOS,NOS level of protein expression and NO level,SBP,DBP,pulse pressure difference were compared before intervention between the two groups,there was no statistically significant difference(P>0.05),prompted two comparable set of baseline data.2.Arg-Ⅱ protein level(1)Intra group comparisons:(1)intervention group: before and after 24 weeks of intervention,Arg-Ⅱ protein levels compared differences in the group had no statistical significance(P=0.068>0.05);(2)control group: before and after 24 weeks of intervention,the level of Arg-Ⅱ protein levels compared differences had no statistical significance(P=0.659>0.05).(2)Inter group comparison: before and after intervention,Arg-Ⅱ protein levels compared differences between groups had no statistical significance(P=0.697>0.05);there was no significant difference between the two groups before and after intervention(P=0.176>0.05).3.NOS protein level(1)Intra group comparisons:(1)intervention group: there was statistically significant difference in the level of NOS protein between the group before intervention and after 24 weeks of intervention(P=0.000<0.05),and the level of NOS protein after intervention was higher than that before intervention;(2)control group: there was no significant difference in the level of NOS protein between before and after 24 weeks of intervention(P= 0.819>0.05).(2)Inter group comparison: after 6 months of intervention,the difference in NOS protein level between the two groups was statistically significant(P=0.012<0.05).The difference between the two groups before and after the intervention was statistically significant(P=0.000<0.05),and the level of NOS protein in the intervention group was higher than that in the control group.4.Activity of NOS protein(1)Intra group comparisons:(1)intervention group: there was statistically significant difference in NOS protein activity between the group before intervention and after 24 weeks of intervention(P=0.000<0.05),and the level of NOS protein activity after intervention was higher than that before intervention;(2)control group: there was no statistically significant difference in the activity of NOS protein before intervention and after 24 weeks of intervention(P=0.110>0.05).(2)Inter group comparison: after 24 weeks of intervention,no significant difference was found in the activity of NOS protein between the two groups(P=0.147>0.05).There was statistically significant difference between the two groups before and after intervention(P=0.000<0.05),and the difference between the two groups before and after intervention was higher than that of the control group.5.NO level(1)Intra group comparisons:(1)intervention group: there was statistically significant difference in NO level between between the group before intervention and the group after intervention(P=0.001<0.05),and the NO level after intervention was higher than that before intervention;(2)control group: there was no significant difference in NO level between before and after 24 weeks of intervention(P=0.493>0.05).(2)Inter group comparison: after 24 weeks of intervention,the difference of NO level between the two groups was statistically significant(P=0.045<0.05).The difference between the two groups before and after intervention was statistically significant(P=0.000<0.05),and the level of NO in the intervention group was higher than that in the control group.6.Blood pressure comparison(1)Intra group comparisons:(1)SBP: SBP of patients in the intervention group every 4weeks after intervention was statistically significant compared with that before intervention(all P=0.000<0.01),while there was no significant difference in SBP between patients in the control group every 4 weeks after intervention and before intervention(all P>0.05).(2)DBP:SBP of patients in the intervention group every 4 weeks after intervention was statistically significant compared with that before intervention(P<0.05 or P<0.01),the control group patients in the intervention in 4 weeks,8 weeks,12 weeks after the DBP difference compared with before intervention had no statistical significance(P>0.05),there was statistically significant difference in the intervention of 16 weeks,20 weeks and 24 weeks compared with before intervention(all P<0.05).(3)pulse pressure difference: there was no statisticallysignificant difference between the pulse pressure difference every 4 weeks after intervention and that before intervention in the intervention group(all P>0.05),and there was no statistically significant difference between the pulse pressure difference every 4 weeks after intervention and that before intervention in the control group(all P>0.05).(2)Inter group comparison: after 12 weeks,16 weeks,20 weeks and 24 weeks of intervention,SBP and DBP of the two groups were statistically significant(P<0.05/P<0.01),and SBP/DBP of the intervention group were lower than those of the control group.(1)SBP:the analysis results showed that SBP of the two groups had statistical significance in time,group,and the interaction effect between time and group(P<0.05/P<0.01).(2)DBP: the analysis results showed that DBP of the two groups had statistical significance in time and group(P<0.05/P<0.01),but no statistical difference in the interaction effect(P>0.05).(3)pulse pressure: the analysis results showed that the pulse pressure difference between the two groups had no statistical significance in terms of time,group and the interaction between time and group(all P>0.05).7.Security analysisDuring the intervention,no adverse events related to Baduanjin exercise intervention,such as muscle spasm and fall injury,were found in the patients.Conclusion1.Baduanjin exercise can improve the expression and activity of NOS protein and promote the synthesis of NO in EH patients.2.Baduanjin exercise can reduce SBP and DBP in EH patients,and its mechanism may be related to the up-regulation of NOS expression in L-Arg/NOS/NO pathway and the promotion of NO production.3.Baduanjin exercise is safe and effective,which can be promoted as an adjuvant therapy in EH patients.
