Serum CTRP1 Level In Patients Of Type 2 Diabetes Mellitus Combined With Coronary Heart Disease And The Correlation Of CTRP1 With Inflammatory Factors

Objective: To analyze the level of C1 q tumor necrosis factor related protein 1(CTRP1)in type 2 diabetes mellitus(T2DM)combined with coronary heart disease(CHD),and to investigate the correlation of CTRP1 with inflammatory factors.Methods: The participants were divided into four groups based on coronary angiography and WTO criteria(1999)for diabetes mellitus: normal control group(n=50),simple CHD group(n=52),simple T2 DM group(n=48),T2 DM combined with CHD group(n=51).The serum levels of CTRP1,APN,CRP,TNF-α,IL-6,IL-8,ICAM-1,VCAM-1 and MCP-1 were detected by ELISA.Results:(1)The level of serum CTRP1 in simple CHD group,simple T2 DM group and T2 DM combined with CHD group were elevated than that of normal control group(17.86±9.52ng/m L,13.12±4.98ng/m L,16.02±14.31ng/m L VS 7.55±4.96ng/m L,all P<0.05).(2)Serum CTRP1 level in simple CHD group was significant positively correlated with Gensini scores(r= 0.330,P<0.001),Logistic regression analysis indicated that CTRP1(OR=1.061,95%CI:1.215-1.726,P<0.001)was the risk factor for simple CHD.Serum CTRP1 level was positively correlated with TNF-α(r=0.399,P=0.003)、 IL-6(r=0.522,P<0.001)、IL-8(r=0.507,P<0.001)、ICAM-1(r=0.492,P<0.001)、MCP-1(r=0.506,P<0.001).(3)Simple T2 DM group’s correlation analysis indicated that serum CTRP1 level was positively correlated with BMI(r=0.269,P=0.008)、FPG(r=0.677,P<0.001)、Hb A1C(r=0.452,P<0.001)、TG(r=0.390,P=0.041),and CTRP1 was positively correlated with CRP(r=0.371,P=0.031)、IL-6(r=0.397,P=0.019)、ICAM-1(r=0.495,P=0.021).The Multiple regression analysis of CTRP1’s indicated that,in simple T2 DM group ICAM-1(β=0.015,P=0.017)、FPG(β=0.981,P=0.004)were the independent influencing factors for CTRP1.(4)T2DM combined with CHD group ’s Logistic regression analysis indicated that CTRP1(OR=1.046,95%CI:1.019-1.306,P=0.007)was the risk factors for T2 DM combined with CHD.CTRP1 was positively correlated with IL-6(r=0.454,P=0.001)、 ICAM-1(r=0.284,P=0.044).Conclusion:(1)Serum CTRP1 level is significantly increased in CHD patients,the level of CTRP1 is positively correlated with the severity of coronary lesions,and is an independent risk factor for CHD.CTRP1 is involved in the process of coronary atherosclerosis,determination of CTRP1 level in serum may be used as a biochemical indicator to reflect the severity of CHD.(2)Serum CTRP1 levels is significantly elevated in patients with T2 DM,CTRP1 may be involved in the pathogenesis of hyperglycemia and T2 DM,but is not the independent risk factor for T2 DM.(3)Serum CTRP1 levels is significantly elevated in patients with T2 DM combined with CHD,the coexistence of CHD and T2 DM cannot further increase the level of serum CTRP1,indicating that the effects of CHD and T2 DM on CTRP1 cannot be superimposed.(4)CTRP1 is an independent risk factor for T2 DM combined with CHD,CTRP1 involves in the inflammatory response of T2 DM and CHD,but the inflammatory factors in T2 DM combined with CHD are not the independent factors of CTRP1.
