Objective:to establish the Turner tooth animal model,including the experimental dog model of periapical periodontitis of deciduous teeth and contusion injury of deciduous teeth.To analyze the role of damps and TLR4 in deciduous teeth contusion and pathogen associated molecular model molecules(PAMPs)in the influence of apicitis on the inherited permanent teeth,and to further explore the mechanism of Turner teeth.In order to provide a new idea for clinical treatment of contusion injury of deciduous teeth,it is necessary to extract the injured deciduous teeth as early as possible.Through the analysis of clinical cases of Turner’s teeth,the etiology and clinical manifestations of Turner’s teeth were studied.Methods:1.the periapical periodontitis model of deciduous teeth was established in nine 35-day-old local dogs.Nine dogs were randomly divided into three groups,three in each group.The left upper and lower incisors and canine of the same dog were the blank control group,without treatment.The right upper and lower incisors and canine were the experimental group.One group of experimental dogs were killed one week,two weeks and three weeks after the pulpotomy,and the animal jaws were taken.2.Twelve 45-day-old local hybrid dogs were selected to establish the model of deciduous teeth contusion.Each dog’s right upper primary incisor and canine gingival setback were extracted half an hour later,which was the extraction setback group;the left upper primary incisor and canine gingival setback were retained,which was the retention setback group;the right lower primary incisor and canine were not treated as the blank control group,12 dogs were randomly divided into four groups,three in each group,and one group was killed at 24 hours,48 hours,72 hours and 96 hours respectively Experimental dogs,animal jaws.3.CBCT was taken for the specimen,and the inherited permanent tooth germ of deciduous teeth was selected under the guidance of CT.Paraffin section was made.The histomorphology of permanent tooth germ was observed by HE staining.The expression of HMGB1 in the contusion injury model of deciduous teeth was detected by immunohistochemistry,and the expression of TLR4 receptor in the periapical periodontitis and contusion injury model of deciduous teeth was detected.4.Thirty cases of Turner’s teeth were collected and analyzed.Results:1.The dog models of contusion and periapical periodontitis of deciduous teeth were established successfully.2.The results of immunohistochemistry showed that TLR4 and HMGB1 were not expressed or occasionally seen in the control group and 24h group,the expression of TLR4 and HMGB1 was significantly increased in 48h group,the strong positive expression remained at a high level at 72h and decreased in 96h group,and the expression of HMGB1 and TLR4 was positively correlated;in the model of deciduous apicitis,TLR4 was not expressed or occasionally seen in the blank group,but weakly positive in 1W group The expression of TLR4 in deciduous teeth contusion injury was positively correlated with that in deciduous teeth apicitis.3.the expression of HMGB1 and TLR4 in extraction and contusion group was weaker than that in retention and contusion group at the same time,the difference was statistically significant.4.The main causes of Turner’s teeth are infection of primary teeth,followed by trauma of primary teeth,enamel defect and discoloration are the most common manifestations of Turner’s teeth,followed by enamel defect,brownish yellow and chalky discoloration.Conclusion:1.The animal model of Turner’s teeth can be successfully reproduced by lipopolysaccharide sealing of canine deciduous teeth and gingival setback of canine deciduous teeth;2.damage related model molecules and their receptors,pathogen related model molecules and receptors participate in the formation of Turner’s teeth;3.local inflammation can be reduced by removing setback into deciduous teeth after setback injury of deciduous teeth.4.The main causes of Turner’s teeth are infection of primary teeth,followed by trauma of primary teeth,and the most common clinical manifestations are enamel defect and discoloration at the same time.
