| ObjectiveThe expression levels of 17 cytokines including macrophage inflammatory protein-3α in gastric cancer serum and their correlation with mi R-10 b were analyzed to screen out factors that are meaningful for the diagnosis of gastric cancer and explore their diagnostic application value.MethodBased on the design of a case-control study,the serum samples of 34 patients with a clear diagnosis of gastric adenocarcinoma by histopathology were selected as the cases,and the serum samples of 80 individuals with normal physical examination in the physical examination center were selected as the controls.Luminex detection technology was used to detect the serum levels of gastric cancer-related cytokines which were selected by literature review in 114 serum samples.A total of 17 kinds of cytokines including macrophage inflammatory protein-3α(CCL20),interleukin-2(IL-2),interleukin-4(IL-4),Interleukin-5(IL-5),interleukin-6(IL-6),interleukin-10(IL-10),interleukin-17(IL-17),tumor necrosis factor-α(TNF-α),interferon-γ(IFN-γ),monocyte chemoattractant protein(MCP)-4(CCL13),interferon-induced T cell alpha chemokine(CXCL11),B-Lymphocyte Chemoattractant(BLC)(CXCL13),bone morphogenetic protein 4(BMP4),bone morphogenetic protein7(BMP7),hepatocyte growth factor(HGF),granulocyte-macrophage colony stimulating factor(GM-CSF),platelet derived growth factor-BB(PDGF-BB)were determined.The serum level of mi R-10 b in the cases was detected by q RT-PCR to explore the relationship between mir-10 b and cytokines in the pathogenesis of gastric cancer.In addition,the serum levels of tumor markers CEA,CA19-9and CA72-4 in the gastric cancer group were detected by electrochemical luminescence method,which was used to compare its diagnostic efficacy with macrophage inflammatory protein-3α(CCL20).Results1.By comparing the expression levels of 17 cytokines in the gastric cancer group and the control group,9 significantly increased cytokines were screened out: CCL20,IL-2,IL-4,IL-6,IL-17,TNF-γ,CCL13,BMP-4 and HGF,with statistically significant differences(P<0.05).2.Analysis of the clinicopathological data of its biological characteristics with gastric cancer patients showed that there was no significant difference in expression levels between the groups of age,gender and Lauren type(P>0.05).Serum levels of IL-4 and TNF-α in the group with tumor diameter greater than5 cm were significantly increased compared with the group with tumor diameter less than 5cm(P<0.05).Compared with the non-metastatic group,the serum levels of IL-10,PDGF-BB in the metastatic group were significantly increased(P<0.05).The level of TNF-α and CCL20 in serum was related to TNM grade: The level of stage III gastric cancer patients was significantly higher than that of stage I patients(P<0.05).3.Further ROC curve analysis and screening indicated that CCL20 had great potential diagnostic value(AUC=0.92),which was better than the tumor markers CEA,CA19-9 and CA72-4(AUC=0.64).6.71pg/ml as the critical point between normal and abnormal,the specificity can reach 76.5% and the sensitivity is 91.3%.The positive predictive value was 78.8%,the negative predictive value was 90.1%,the positive likelihood ratio was 8.74,the negative likelihood ratio was 0.26,and the diagnostic accuracy was 86.84%.4.Spearman correlation analysis of 17 cytokines and mi R-10 b showed poor correlation.ConclusionsThe expression level of CCL20 in the gastric cancer group was significantly higher than that in the control group,and the difference was statistically significant.Through ROC curve analysis,it was found that the sensitivity of CCL20 was significantly better than the commonly used tumor markers CEA,CA19-9 and CA72-4,and the specificity was also maintained at a good level,which is expected to become a new marker of gastric cancer. |
