Abstract/Summary:
Objective:To understand the self-perceptions of aging,self-management motivation,self-management status and their differences in general demographic characteristics of elderly patients with hypertension in the community;to analyze the relationship among self-perceptions of aging,self-management motivation and self-management level of elderly patients with hypertension;to explore the influence of self-perceptions of aging and self-management motivation on self-management of elderly patients with hypertension. To explore the influence of self-perceptions aging and self-management motivation on self-management of elderly patients with hypertension, and provide the related research and nursing intervention of elderly patients with hypertension. Methods:Using convenient sampling method, the elderly hypertensive patients with informed consent and voluntary participation were collected during the outpatient follow-up of hypertension patients who had established health records in a community health service center in Jilin province.General information,self-perceptions of aging, hypertension self-management motivation and hypertension self-management were investigated with structure questionnaire.SPSS24.0 and Amos20.0 were used for statistical description,t-test,ANOVA,pearson correlation analysis, multiple linear regression and path analysis.Results:(1)the average age of 393 subjects was 68.77±6.48 years old, including 227(57.8%)females,278(70.7%)Han and other nationalities,322(81.9%)urban residents,334(85%)spouses,the education level of junior middle school education accounted for the largest proportion(32.6%),and the average monthly income of family was 1000-2999 yuan were 219(55.7%),and 339(86 3%)medical insurance, blood pressure control was 289(73.5%).blood pressure frequency was measured at least once a week was 148(37.7%).sleep quality was 174(44.3%),health self-evaluation was poor was 162(41.2%).in other chronic diseases, there was 168(42.7%)with one chronic disease.(2)the average score of self-perceptions of aging was 3.1±0.39;the average score of self-management motivation was 90.67±10.71,and the self-management motivation was in the middle level;the average score of self-management was 71.03±17.66.In general demographic characteristics, different gender, age, nationality, marital status, education level, family per capita monthly income, medical expenses, blood pressure control status, sleep status, health self-assessment,and other chronic diseases have significant differences in self-perceptions of aging,self-management motivation and self-management(P<0.05).(3)Correlation analysis showed that the total score of self-perceptions of aging was positively correlated with the total score of self-management motivation(r=0.698,P<0.01),the total score of self-perceptions of aging was positively correlated with the total score of self-management(r=.420,P<0.01),and the total score of self-management motivation was positively correlated with the total score of self-management(r=0.692,P<0.01).(4)multiple linear regression analysis showed that after controlling the general demographic characteristics, the explanatory power of self-perceptions of aging on self-management of the subjects increased by 10%,the explanatory Power of self-management motivation on self-management behavior of the subjects increased by 21%,and the explanatory power of all variables accounted for 53%of the total variation. The explanatory Power of the three models reached a significant level(P<0.05).(5)the results of path analysis showed that self-management motivation had a direct positive impact on the self-management of elderly patients with hypertension;in the sub dimension of self-management motivation, emotional well-being as an indicator variable directly affected self-management,and emotional well-being had the strongest impact on self-management;self-perceptions of aging had a negative impact on self-management under the role of self-management motivation.Conclusions:(1)The elderly patients with hypertension are in the middle level of self-perceptions of aging,self-management motivation and self-management.Female,younger age,Han nationality and other nationalities, spouse, higher education level, family monthly income of 5000 yuan or more, medical insurance, better blood pressure control, good sleep quality, better health self-evaluation,with three chronic diseases, the subjects had higher levels of self-perceptions of aging and self-management motivation,as well as better self-management.(2)There was a positive correlation between self-perceptions of aging,self-management motivation and self-management The younger the age,the better the health self-evaluation,the positive self-perceptions of aging and the high motivation of self-management,the better the self-management of the elderly patients with hypertension.(3)Self-management motivation has a direct positive impact on the self-management of elderly patients with hypertension. In the motor dimension of self-management,emotional well-being as an indicator variable directly affects self-management.Self perceived aging has a negative impact on self-management under the role of self-management motivation.
