| ObjectiveTo explore the preventive effect of Modified Wuling powder on macular edema after phacoemulsification in diabetic patients with cataract.MethodThis study was a prospective,single-center randomized clinical observational study.The study subjects selected 96 patients(99 eyes)with diabetes and cataract who underwent cataract surgery at Zhongshan Hospital of TCM affiliated to Guangzhou University of Traditional Chinese Medicine from May 2019 to December 2019.They were randomly divided into two groups,48 patients(50 eyes)in the control group and 48 patients(49 eyes)in the study group.All cases were treated with phacoemulsification combined with intraocular lens implantation.The control group was treated with Tobra Dex,0.5%levofloxacin eye drops(clopbitol),0.1%sodium hyaluronate eye drops(hycosan)for 1 week,1-2 drops/time,4 times/day.On the basis of the same treatment,the study group was given modified Wuling Powder decoction orally.Observe and collect the best corrected visual acuity(BCVA),intraocular pressure(IOP)before operation,1 week and 1 month after surgery.The incidence of macular central retinal thickness(CMT)and macular edema detected by optical coherence tomography(OCT)were analyzed and compared between the two groups.Results1.BCVA of the two groups was significantly improved one week after operation(P<0.05).There was no significant difference in BCVA between the control group and the study group 1 week after operation(P>0.05).In the 1 month after operation,the improvement of BCVA in the control group had no statistical significance compared with the 1 week after operation(P>0.05).The improvement of BCVA in study group was statistically significant compared with 1 week after operation(P<0.05).Compared with the control group,the visual acuity of the study group was better than that of the control group(P<0.05).2.The overall trend of postoperative CMT in both groups was increased.The CMT in the control group was significantly higher one week after operation than before operation(P<0.05),and was significantly higher one month after operation than before operation and one week after operation(P<0.05).There was no significant difference in the increase of CMT in the study group one week、one month after operation compared with that before operation(P>0.05).There was no significant difference in CMT between the control group and the study group before and 1 week after operation(P>0.05).One month after operation,CMT in control group was significantly higher than that in study group(P<0.05).3.The overall trend of IOP in the control group and the study group decreased after surgery(P<0.05).The IOP in the control group and the study group was significantly lower than that before surgery at 1 week and 1 month after surgery(P<0.05).There was no significant difference in intraocular pressure between the control group and the study group after surgery.(P>0.05).4.the incidence rate of macular edema in the control group was 12.0%,while that in the study group was 4.0%.There was no statistical significance between two groups(P>0.05).there was no significant difference in the incidence rate of macular edema between the two groups one week and one month after operation(P>0.05).Conclusions1.Modified Wuling Powder can effectively improve the visual function of diabetic patients 1 month after PHACO operation.2.Modified Wuling Powder can effectively reduce CMT increase in diabetic patients 1 month after PHACO operation.3.Modified Wuling Powder may reduce intraocular pressure of diabetic patients 1 week and 1 month after PHACO operation to a certain extent.4.Based on the physical distribution of diabetic patients with cataract and the characteristics of macular edema in traditional Chinese medicine,this study suggests that the modified Wuling Powder may have certain preventive effect on macular edema in diabetic patients after PHACO operation. |
