| BackgroundChronic Obstructive Pulmonary Disease,referred to as COPD,is a systemic chronic inflammatory response disease characterized by persistent airflow limitation.Although its affected organ is the lung,but it is often combined with other system diseases.Anxiety and/or depression is one of the extrapulmonary complications of COPD.Researchers are paying more and more attention to anxiety and/or depression,which is one of the extrapulmonary complications of COPD.Repeative aggravation of the disease,progressive decline of lung function,reduced quality of life,other complications,economic burden of long term medical treatment,and lack of social care make the patients feel anxiety and/or depression.These harmful emotion are easily ignored by patients or people around them,which is the difficulty of chronic disease management of COPD.The main manifestations of the patients are feeling depression,lack of interest,feeling feeble,somnipathy etc.These anxiety and/or depression in a long time will also lead to repeated aggravation of patients’ condition and decline in quality of life.In Most of the researches on COPD with anxiety and/or depression begin with the pharmacotherapy of treatment of traditional Chinese medicine or western medicine.There are few studies on the curative effect of traditional Chinese medicine therapies on patients with COPD complicated anxiety and/or depression.The study treat patients with COPD complicated anxiety and/or depression by "fiery dragon"moxibustion,one of the traditional Chinese medicine therapies,in order to improve the clinical symptoms of patients,decrease anxiety and/or depression and improve the quality of life.ObjectiveThe study is to exploring the effect of "fiery dragon" moxibustion on the improvement of clinical symptoms and quality of life of patients with chronic obstructive pulmonary disease complicated anxiety and/or depression.MethodsThis had selected 53 patients from the inpatient department of the Department of Respiratory,Fangcun Hospital of Guangdong Provincial Hospital of Chinese Medicine during June 2018 to January 2020 with chronic obstructive pulmonary disease combined with anxiety and/or depression.The patients were belong to traditional Chinese medicine syndromes types of the deficiency of both the lung and the spleen syndrome.Patients were randomly divided into"fiery dragon" moxibustion group and routine group.The "fiery dragon"moxibustion group included 26 cases,1 case falling off.Routine group included 27 cases,2 cases falling off.Routine group was given Routine treatment such as oxygen inhalation,atomization,anti inflammation and anti infection etc.The "fiery dragon" moxibustion group was given "fiery dragon" moxibustion treatment on the basis of routine treatment.Treatment course 1 week.Before and after treatment,we evaluate the the traditional Chinese medicine symptom score,self-rating anxiety scale(SAS),self-rating depression scale(SDS),chronic obstructive pulmonary disease assessment test(CAT),6-minute walk test(6MWT),and lung function,etc.During the treatment,adverse reactions will be recorded.Results(1)Traditional Chinese medicine symptom score:After treatment,the patients of the "fiery dragon" moxibustion group and the routine group in cough,sputum,shortness of breath and chest tightness were improved compared with before treatment,compared with before treatment,P<0.05,the difference was statistically significant.However in the anxiety and/or depression symptoms such as palpitations,upset,insomnia,dreams,worries,the score in "fiery dragon" moxibustion group decreased(Compared with before treatment,P<0.05,the difference was statistically significant).while the score change in the routine group was not obvious,compared with before treatment P>0.05,the difference was not statistically significant.(2)Traditional Chinese medicine efficacy:The total scores of TCM symptom scores in both "fiery dragon" moxibustion group and the routine group were decreased before treatment,compared with before treatment P<0.05,the difference was statistically significant.After treatment,the total score of TCM symptom scores of "fiery dragon" moxibustion group is lower than routine group.Compared with P<0.01,the difference was statistically obvious significance.In terms of the total curative effect of Chinese medicine symptoms,the treatment effective rate of the "fiery dragon" moxibustion group was 92%,and the treatment effective rate of the routine group was 28%.The difference of the total effective rate between two groups was P<0.05,the difference was statistically significant.(3)Anxiety and depression:The scores of the self-rating anxiety scale(SAS)and the self-rating depression scale(SDS)of the "fiery dragon"moxibustion group were were significantly reduced compared with before treatment(P<0.05)while the self-assessment of anxiety scale(SAS)score and self-assessment of depression scale(SDS)score of patients in the routine group were not significantly changed compared with those before treatment.Compared with before treatment(P>0.05).(4)Quality of life:The "fiery dragon" moxibustion group had significantly lower CAT scores after treatment than before,compared with before treatment P<0.01,which was statistically significant.The 6-minute walk test(6MWT)The walking distance was significantly increased,compared with before treatment P<0.01 which the difference was statistically significant.However,there are no significant change in CAT score and 6-minute walking test(6MWT)walking distance in the routine group,compared with before treatment P>0.05 which the difference was not statistically significant.(5)Pulmonary function:The percentage of forced expiratory volume in the first seconds ccounted for the predicted value(FEV1%)in the "fiery dragon"moxibustion group and the routine group before and after treatment has no significant difference(P>0.05).ConclusionThe "fiery dragon" moxibustion could reduce anxiety and/or depression symptoms such as palpitations,insomnia and dreams,worries and anxious in patients with chronic obstructive pulmonary disease complicated anxiety and/or depression and could improve the quality of life of such patients to a certain extent. |
