| Objective: To analyze the prognostic factors affecting the survival of NSCLC spinal metastases and to evaluate the accuracy of scoring systems in the survival of spinal metastatic tumors in NSCLC.Methods: We retrospectivelyreviewedthedataof 63 patients treatedfor with NSCLC spinal metastases between January 2013 and December 2017.Age,sex,paralysis state,general condition,histological subtypes,the number of spinal,extra-spinal bone,visceral metastasis,NSE,CEA,CA19-9,calcium,alkaline phosphatase,albumin,surgery,percutaneous vertebroplasty,chemotherapy,radiotherapy and molecule-targeting drug treatment were included in the factors affecting the prognosis.the log-rank test and multivariate Cox proportional hazards models were fit to determine the features associated with survival.Scores were calculated for Tomita,Modified Baur,van der Linden,Revised Tokuhashi and Tokuhashi lung cancer scoring systems.Kaplan-meier survival curve was generated by each scoring system.Prognostic value was determined by comparison of predicted and actual survival.Results: Univariate analysis revealed that the following features were associated with survival:general condition,CA19-9,spinal external bone metastasis,chemotherapy,radiotherapy and molecule-targeting drug treatment.The results of multivariate analysis showed that general condition and molecule-targeting drug treatment were the independent factors affecting the prognosis.Except for the Tokuhashi lung cancer scoring system,there was no statistically significant difference in prognosis between the other scoring systems.The accuracy of Tokuhashi lung cancer scoring system was 66.7%,while the accuracy of other scoring systems was less than Conclusion: In patients with NSCLC spinal metastases,survival was associated with general condition,CA19-9,spinal external bone metastasis,chemotherapy,radiotherapy and molecule-targeting drug treatment,among which general condition and molecule-targeting drug treatment are independent factors affecting the prognosis.Tokuhashi lung cancer score system has the highest accuracy for spinal metastatic tumors of NSCLC. |
