The Study On The Clinical Effect Yihunqiangpo Decoction In Treating Alzheimer’s Disease With Deficiency Of Hun And Po On Five Spirits Theory

Objective: Based on the previous research of research group,it is found that the pathogenesis of Alzheimer disease(AD)can be better interpreted from the perspective of imbalance of Hun and Po,the pathogenesis of Alzheimer’s disease with deficiency of Hun and Po is the most obvious.Through the extension of the previous topic,combined with the clinical experience of the tutor based on the theory of "five spirits" of TCM in the diagnosis and treatment of AD,This paper evaluate disease from the level of the pathogenesis with Deficiency of Hun and Po and establish treatment targets,observe the the clinical efficacy of Yihunqiangpo Decoction on the treatment of AD with deficiency of Hun and Po,establishing a new path of AD diagnosis and treatment of five spirits.Method: Outpatients and inpatients were selected in the affiliated hospital of Shandong University of Traditional Chinese Medicine from January 2018 to December2019,sixty patients with mild-to-moderate deficiency of Hun and Po type AD who met the inclusion criteria were divided into a test group and a control group of 30 cases according to the random number table.The test group was treated with integrated traditional Chinese and western medicine,given Yihunqiangpo decoction and donepezil hydrochloride tablets.The control group was treated with simple western medicine,given donepezil hydrochloride tablets.The treatment period is 8 weeks.Before and after the treatment,AD patients were evaluated by the self-made Chinese medicine five spirits "Deficiency of Hun and Po" symptom rating scale,Minimum Mental State Examination(MMSE),and Activity of Daily Living(ADL)assessment scale.Observe the improvement of the MMSE,ADL and five spirits of TCM syndrome scores before and after treatment in both groups of patients,at the same time assess the safety of medication,record possible adverse reactions,organize and analyze clinical data,determine the clinical efficacy of the two groups of patients after treatment,and draw conclusion.Results:1.There was no significant difference in general data(gender,age,course of disease,education level,etc.)and CDR,MMSE,ADL and five spirits of TCM syndrome scores before treatment(P> 0.05)between the two groups,and it was comparable.2.TCM Clinical efficacy efficacy determination:(1)Control group total 30 cases,cure 0 case,excellent effective 2 cases,effective 15 cases,invalid 13 cases,TCM syndromes total effective 56.67%.Test group total 30,3clinical cures,cure 0 case,excellent effective 6 cases,effective 20 cases,invalid 4cases,TCM syndromes total effective 86.67%.The clinical efficiency of TCM syndromes in the test group is significantly higher than that in the control group(P<0.05).(2)"Deficiency of Hun and Po" of TCM syndrome scores and various components:After treatment,"Deficiency of Hun and Po" of TCM syndrome scores of both groups were lower than before(P<0.05).The improvement in the test group was better than the control group,with statistical significance(P<0.05).The control group’s TCM symptom score improved after treatment(P<0.05)in "lazy thinking,unresponsiveness";"restless limbs,uneasy sleep";"not daring to face others";"attention range narrowed,turning a blind eye to surrounding things";"daytime sleepiness";"slow language,low voice";"unable to remember recent events";"slurred speech";"can’t recall what was just said or just done".After treatment,the scores of TCM syndromes in the test group were improved compared with the previous ones(P<0.05);and the improvement in the test group was better than the control group,with statistical significance(P<0.05)3.Western medical effect determination:After treatment,the MMSE score and ADL score of the control group and the test group were within the group comparison and the group comparison P was less than 0.05,which was statistically significant,indicating that the two groups of patients have improved cognitive function and daily life ability after treatment,and the improvement in the test group is significantly better than that in the control group4.Safety assessmentRoutine inspection and laboratory index detection showed no significant abnormalities.Conclusion: Yihunqiangpo decoction combined with donepezil hydrochloride tablets in the treatment of AD with deficiency of Hun and Po has a significant clinical effect.Compared with simple Western medicine treatment,it can further improve the cognitive function,daily life ability and psycho-psychological behavior abnormal symptoms of AD patients.It can really improve the curative effect of the Five spirits of TCM syndrome,safety in clinical,was worthy of clinical promotion and application.
