Study On The Constitution Distribution And Risk Factors Of Persistent Postural-perceptual Dizziness In Traditional Chinese Medicine

Object: Through the identification of traditional Chinese Medicine Constitution of patients with persistent postural-perceptual dizziness(PPPD),observe and analyze the characteristics of the disease and the distribution of traditional Chinese medicine constitution,explore the risk factors that may affect the pathogenesis of PPPD,and provide reference for the prevention and treatment of this disease.Methods: From March 2018 to December 2019,202 PPPD patients were collected from the outpatient department of encephalopathy of the Affiliated Hospital of Shandong University of traditional Chinese medicine,recording the general situation of patients’ sex,age,past history,etc,and a special person was assigned to assess the patients’ constitution according to the scale of classification and determination of constitution of traditional Chinese medicine;a database was established by using Excel software,and spss20.0 software was used for statistical analysis to explore the characteristics of PPPD,the law of TCM Constitution distribution and risk factors.Results:(1)General data: the sex and age distribution of PPPD patients were statistically significant(P < 0.01).The proportion of male and female patients was 1:2.42,and 60-69 years old was the age of high incidence,accounting for 32.2% of the total number of patients.Nonparametric test(z=-2.37,P = 0.018 < 0.05)was performed on the age distribution of male and female patients with PPPD.(2)TCM Constitution Type: PPPD patients’ TCM constitution type is: Qi Stagnation(17.9%)> Qi deficiency(15.8%);Yang deficiency(15.4%);Yin deficiency(14.7%);phlegm dampness(12.0%);peace quality(9.5%);blood stasis quality(8.7%);damp heat quality(4.6%);special quality(1.4%).There was statistical difference in the distribution of patients’ constitution between genders(P = 0.014 < 0.05).The common constitution of male patients was phlegm dampness,yin deficiency and Qi deficiency,while that of female patients was qi depression,Yang deficiency and Qi deficiency.(3)Risk factors: among the PPPD patients,26 were smokers and 19 were drinkers,accounting for 12.8% and 9.4% of the total;the most common comorbidity was psychological anxiety,accounting for 48.01% of the total;the second was hypertension and migraine,accounting for 32.67% and 17.32%.Among the patients with anxiety comorbidity,the number of patients in the obvious anxiety group was 68,accounting for 70.1% of the patients with anxiety,followed by 15 and 14 in the anxiety group and the severe anxiety group,respectively.There was statistically significant difference in the number of patients in different anxiety groups by chi square test(P = 0.045< 0.05).Conclusions:(1)PPPD is more common in the elderly and women.The onset age of PPPD in men is earlier than that in women.(2)The common constitution types of PPPD patients are qi stagnation,Qi deficiency and yang deficiency.The distribution of constitution types is related to gender.Most of the male patients are phlegm dampness,and most of the female patients are qi stagnation.(3)Anxiety state,hypertension and migraine may be the risk factors of PPPD patients.Smoking,drinking and the disease have little correlation.Obviously,anxiety patients are more likely to suffer from PPPD than anxiety patients.
