| Objective:To compare the therapeutic effect of Xingqi Huoxue decoction and sodium aescinate injection and conventional treatment combined with surgery on B3 ankle fracture.Provide a safer,reasonable and efficient treatment ideas and methods for clinical treatment of ankle fractures,and promote and share their treatment experience.Methods:A total of 90 patients with type B3 ankle fractures who underwent surgery from September 2017 to September 2019 in the Department of Orthopedics,Affiliated Hospital of Shandong University of Traditional Chinese Medicine were selected for study.According to the random number table method,the subjects were divided into experimental group,control group and blank group.The three groups of patients were given conventional drugs and surgery to treat ankle fractures.Among them,the experimental group applied Xingqihuoxue decoction on the basis of basic treatment,the control group applied sodium aescinate for injection on the basis of basic treatment,and the blank group only applied conventional treatment.All cases were followed up for 6-12 months,Compare the swelling values of the three groups of patients on day 0 and day 2 and day 5,and day 1 and day 4 and day 7 after admission,the VAS pain scores on day 0 and day 2 and day 5,and day 1 and day 4 and day 7 after admission,and the C-reactive protein on day 1 and day 4 after admission and day 1 and day 4 after surgery Value,waiting time for surgery and AOFAS ankle-hindfoot score at 6 months after surgery,.The data was processed by SPSS26.0 statistical software.Result:The three groups of data were compared at age,gender,swelling value on day 0 and 1 day after admission,VAS pain score,C-reactive protein value on day 1 after admission and day 1 after operation(P>0.05),the difference is not statistically significant and the three sets of data are comparable.(1).From the data of swelling value,the data of the three groups of patients were compared on day 0 after admission and day 1 after surgery.(P<0.05),Comparison among groups,the data of the three groups on the 2nd and 5th day of admission and the 4th and 7th day after operation,P<0.05,which was statistically significant.From the average value of each group at different time points,the average value of the swelling value of each group increased on the second day after admission,and the swelling value on the 5th day of admission and the 4th and 7th days after the surgery decreased.Combining the results of pairwise comparison(P<0.05),the average swelling value of the experimental group was smaller than that of the control group and smaller than that of the blank group.It shows that oral Xingqi Huoxue decoction has a significant effect on improving the early swelling of B3 ankle fractures,and is superior to sodium aescinate injection.(2).From the VAS pain score data,compare the data of the three groups of patients after admission and at various time points after surgery(P<0.05),Comparison of data between the three groups on the 2nd and 5th day of admission and the 4th and 7th day after operation(P<0.05),With the extension of time,the average pain value after admission and postoperatively decreased,According to the results of the comparison between the two groups(P<0.05),the decrease of VAS pain score in the experimental group was greater than that in the control group,It shows that oral Xingqi Huoxue decoction has a significant effect in relieving early pain of B3 ankle fracture,and it is better than sodium aescinate injection.(3).rom the results of C-reactive protein value,the paired sample T test was used to compare the data of three groups of patients at different time points after admission and postoperatively(P<0.05).Comparing the three groups of data on the 4th day of admission and the 4th day after surgery,(P<0.05),With the prolongation of time,the average value of each group of data after admission and postoperatively decreased,According to the results of the comparison between the two groups(P<0.05),the average value of C-reactive protein in the experimental group was smaller than that in the control group and smaller than that in the blank group,It shows that oral Xingqi Huoxue Decoction has a significant effect on the early anti-inflammatory effect of B3 ankle fractures,and it is better than sodium aescinate injection.(4).From the results of the waiting time of surgery,the three sets of data were subjected to rank sum test P<0.05,And the comparison result of rank difference between two pairs P<0.05,It shows that oral Xingqi Huoxue decoction can shorten the waiting time of B3 ankle fracture surgery.(5).From 6 months after operation,the comparison of the AOFAS ankle-hindfoot scores between the three groups(P>0.05)showed no significant difference.Conclusion:The application of Xingqi Huoxue Decoction combined with surgery for B3 ankle fracture has achieved good results.especially in reducing swelling,reducing pain,reducing inflammatory indicators and shortening the waiting time for surgery,However,it had little effect on the recovery of ankle function 6 months after operation.Therefore,the application of Xingqi Huoxue Decoction combined with surgery in the treatment of B3 ankle fractures in the early stage is an ideal choice. |
